目的 :探讨声学定量 (AQ)技术检测小儿扩张型心肌病患者的左室功能的应用价值并评价患儿的心功能改变。方法 :应用声学定量技术、二维越声心动图 (2 - DE)及多普勒越声心动图 (PDE)检测 16例扩张型心肌病 (DCM)患儿的左室功能 ,并对两种方法进行对比研究 ,探讨二者间的一致性。结果 :本组患儿 93%可进行 AQ技术检查 ;后者可取代 2 - DE测定小儿 DCM的左室面积指标、射血分数 ,但两者测定的左室容积指标一致性尚欠佳 ;DCM患儿的左室舒张末期及收缩末期面积增大 ,面积变化分数、射血分数降低 ,显示左室收缩功能受损 ;对扩张型心肌病患儿左室舒张功能检测表明 ,AQ技术敏感性高于 PDE,尤其 PDE二尖瓣血流频普显示假性正常时。AQ技术与 PDE的联合应用 ,更有利于小儿左室舒张功能的检测。结论 :AQ技术可实时、快速、可靠地进行小儿 DCM心功能检测 ,为临床药物的选择、疗效和预后的判断提供依据
Objective: To investigate the value of acoustic quantification (AQ) in detection of left ventricular function in children with dilated cardiomyopathy and to evaluate the changes of cardiac function in children. Methods: The left ventricular function in 16 children with dilated cardiomyopathy (DCM) was measured by acoustic quantification, two - dimensional echocardiography (2 - DE) and Doppler echocardiography (PDE) Methods of comparative study to explore the consistency between the two. Results: Ninety - three percent of the children in this group were eligible for AQ test. The latter could replace the 2 - DE in determining left ventricular area index and ejection fraction in children with DCM, but the consistency between the two indexes was not good. DCM Diastolic left ventricular end-diastolic and end-systolic area increased, area change score, ejection fraction decreased, showing left ventricular systolic dysfunction; left ventricular diastolic function in children with dilated cardiomyopathy test showed that AQ high sensitivity In PDE, especially PDE mitral flow frequency Pseudo-normal show. AQ technology and PDE joint application, more conducive to the detection of left ventricular diastolic function in children. Conclusion: AQ can detect cardiac function in children in real time, quickly and reliably, and provide the basis for clinical drug selection, curative effect and prognosis
