目的综合评价肺炎衣原体(Chlamydia pneumoniae,Cpn)感染与肺癌之间的关系,为肺癌病因研究和临床诊治提供科学依据。方法检索Medline数据库、中国生物医学文献数据库、中国知识资源总库、维普资讯网、万方数据知识服务平台等多个中英文数据库,收集Cpn感染与肺癌的病例对照研究文献,同时利用所查文献及相关综述的参考文献、Google学术搜索进行滚动搜索补遗,应用Stata 12.0软件进行meta分析。结果共纳入12篇文献,包括9篇英文文献和3篇中文文献,累计2 557例病例人群和2 756名对照人群;meta分析结果显示,Cpn Ig A抗体阳性和Cpn Ig G抗体阳性与肺癌的合并OR值分别为2.025(95%CI=1.376~2.982)和1.774(95%CI=1.251~2.516),吸烟者Cpn Ig A和Cpn Ig G阳性与肺癌的合并OR值分别为2.372(95%CI=1.463~3.847)和1.908(95%CI=1.112~3.275),Cpn Ig A≥16、Cpn Ig A≥64和Cpn Ig G≥16与肺癌的合并OR值分别为1.180(95%CI=1.029~1.353)、1.836(95%CI=1.098~3.068)和1.608(95%CI=1.022~2.531)。结论 Cpn感染与肺癌发生有关,Cpn感染可增加吸烟者患肺癌的风险。
Objective To evaluate the relationship between Chlamydia pneumoniae (Cpn) infection and lung cancer and provide a scientific basis for the etiological study and clinical diagnosis and treatment of lung cancer. Methods A number of Chinese and English databases including Medline database, Chinese Biomedical Literature Database, China Knowledge Resources Database, VIP Information Network and Wanfang Data Knowledge Service Platform were collected to collect case-control studies on Cpn infection and lung cancer. At the same time, And related reviews, Google Scholar scrolling search addendum, Stata 12.0 software for meta-analysis. Results A total of 12 articles were included, including 9 articles in English and 3 articles in Chinese. A total of 2 557 cases and 2 756 controls were included in the meta-analysis. The results of meta-analysis showed that Cpn Ig A and Cpn Ig G antibodies were positively correlated with lung cancer The combined odds ratios (ORs) were 2.025 (95% CI = 1.376-2.982) and 1.774 (95% CI = 1.251-2.516) respectively. The combined odds ratio of Cpn Ig A and Cpn Ig G positive patients with lung cancer was 2.372 (95% CI = 1.463 ~ 3.847) and 1.908 (95% CI = 1.112 ~ 3.275). The combined OR of Cpn Ig A≥64, Cpn Ig A≥64 and Cpn Ig G≥16 with lung cancer were 1.180 (95% CI = 1.353), 1.836 (95% CI = 1.098-3.068) and 1.608 (95% CI = 1.022-2.531). Conclusion Cpn infection is associated with the development of lung cancer. Cpn infection may increase the risk of lung cancer in smokers.
