[目的]了解苏州市高技能人才高血压和糖尿病医疗费用支出的变化趋势,为该人群相关慢性疾病的防治及医疗经费增长的控制提供依据。[方法]从苏州医疗保险信息数据库中选取苏州城区机关企事业单位参保的高级技工47 129人为研究对象,收集该人群2001—2012年高血压、糖尿病门诊与住院的医疗数据,并进行统计分析。[结果]12年间,高血压现患3 539人(占7.51%)、糖尿病832人(占1.77%)。高血压、糖尿病患者门诊及住院总医疗费用分别为1.769 87亿元、1.291 84亿元,分别占该人群总医疗费用的14.27%和10.42%。糖尿病患者人均门诊及住院医疗费用高于高血压患者(P<0.01),男性人均费用高于女性(P<0.01)。高血压患者中男性人均门诊医疗费用高于女性(P<0.01),但不同性别患者的住院费用差异无统计学意义(P>0.05)。高血压、糖尿病医疗总费用的年均增长率分别为21.95%和19.28%。两种疾病年医疗总费用及其增长速度总体呈逐年上升趋势。[结论]高血压、糖尿病的医疗费用在苏州市高技能人才医疗总经费中所占比例较大,且呈逐年上升趋势。
[Objective] To understand the trend of expenditure on hypertension and diabetes mellitus in high-skilled personnel in Suzhou and provide the basis for the prevention and control of chronic diseases and the increase of medical expenses in this population. [Methods] A total of 47 129 senior technicians insured by enterprises and institutions in Suzhou urban area were selected from Suzhou medical insurance information database, and the medical data of hypertension, diabetes clinic and hospitalization in this population from 2001 to 2012 were collected and statistically analyzed . [Results] There were 3 539 hypertensive patients (7.51%) and 832 diabetic patients (1.77%) in 12 years. Hypertension, diabetes clinics and hospitalization total medical expenses were 1.769 8.7 billion yuan, 1.29184 trillion yuan, accounting for 14.27% and 10.42% of the total medical costs of the population. The per capita outpatient and inpatient costs of diabetes patients were higher than those of hypertensive patients (P <0.01). The per capita cost of men was higher than that of women (P <0.01). Outpatient medical cost per male was higher than that of female in hypertensive patients (P <0.01), but there was no significant difference in hospitalization expenses among different sexes (P> 0.05). The average annual growth rate of the total cost of hypertension and diabetes mellitus was 21.95% and 19.28% respectively. The total annual medical expenses and the growth rate of the two diseases have been increasing year by year. [Conclusion] The medical costs of hypertension and diabetes account for a large proportion of the total funds for the medical treatment of highly skilled personnel in Suzhou City, and have been increasing year by year.
