目的 研究在彩色多普勒超声造影中肝实质性占位血流信号的改变。方法 选择经手术及病理证实的 6 1例肝占位性病变 ,其中肝恶性肿瘤 42例 ,良性病变 19例 ,均经肘静脉注射浓度为 40 0mg/ml的Levovist 6~ 7ml后 ,研究肝良恶性肿瘤中彩色多普勒信号的增强情况。结果 造影前、后彩色多普勒血流显像显示有彩色血流信号的肝实质性占位 ,良性分别为 5 2 .6 % (10 /19)及 73 .6 % (14 /19) ,恶性分别为 80 .9% (34/42 )及 10 0 % (42 /42 )。平均开始增强时间、峰值出现时间、增强总持续时间良性占位分别为 37s、92s、496s ,恶性占位分别为 2 9s、78s、40 5s ,良恶性比较差异有显著性意义。结论 Levovist能明显增强肝实质性占位病变中的彩色多普勒血流信号 ,肝恶性肿瘤的增强时间较良性肿瘤明显缩短 ,可以帮助鉴别诊断。
Objective To study the changes of blood flow signal of liver mass occupying in color Doppler ultrasonography. Methods Sixty-one cases of hepatic space-occupying lesions confirmed by operation and pathology were selected, including 42 cases of hepatic malignancies and 19 cases of benign lesions. All of them were injected with 6 to 7 ml of Levovist at a concentration of 400 mg/ml through the cubital vein. Color Doppler signal enhancement in malignant tumors. Results The color Doppler flow imaging before and after angiography showed a substantial hepatic mass occupying the color flow signal, with benign rates of 52.6% (10/19) and 73.6% (14/19) respectively. Malignancy was 80.9% (34/42) and 100% (42/42), respectively. The average benign increase time, peak appearance time, and total duration of enhancement were 37s, 92s, and 496s respectively. The malignant occupations were 29s, 78s, and 40 5s, respectively. The difference between benign and malignant was significant. Conclusion Levovist can significantly enhance the color Doppler flow signal in liver mass lesions. The enhancement time of hepatic malignancy is significantly shorter than benign tumors, which can help differential diagnosis.
