目的对比PCR-线性探针杂交酶显色法(简称线性探针法)与BACTEC MGIT960液体培养及药敏在结核分枝杆菌检出率的差异,评价两种方法检测结核分枝杆菌对利福平和异烟肼耐药性的应用效果。方法选取医院结核科2013年8月-2015年5月369例确诊及疑似结核病的住院患者送检标本,同时采用线性探针法和BACTEC MGIT960对其进行结核分枝杆菌及利福平和异烟肼耐药性检测,数据采用SPSS 13.0软件进行统计分析。结果线性探针法检测结核分枝杆菌对利福平耐药性的灵敏度为90.4%、特异度为97.3%、Kappa值为0.872;对异烟肼耐药性检测的灵敏度为86.6%、特异度为96.3%、Kappa值为0.841,两种方法在利福平和异烟肼耐药性检测中具有较好的一致性。结论 PCR-线性探针杂交酶显色法是一种快速、准确的结核分枝杆菌及其对利福平和异烟肼耐药性的诊断方法,对耐多药结核病的快速诊断具有显著的优点。
OBJECTIVE To compare the detection rates of Mycobacterium tuberculosis between PCR-linear probe hybridization assay (referred to as linear probe method) and BACTEC MGIT960 liquid culture and drug susceptibility, and to evaluate the difference between two methods for detecting Mycobacterium tuberculosis Therapeutic effect of isoniazid resistance. Methods A total of 369 hospitalized patients with confirmed and suspected tuberculosis from August 2013 to May 2015 in our hospital were enrolled in this study. Mycobacterium tuberculosis, rifampin and isoniazid were tested by linear probe method and BACTEC MGIT960 Drug resistance test, the data using SPSS 13.0 software for statistical analysis. Results The sensitivity of Mycobacterium tuberculosis to rifampicin resistance was 90.4%, specificity was 97.3%, Kappa was 0.872, and the sensitivity to isoniazid resistance was 86.6%. Specificity 96.3%, Kappa value of 0.841, the two methods in rifampicin and isoniazid resistance test has good consistency. Conclusion The PCR-linear probe hybridization assay is a rapid and accurate method for the diagnosis of Mycobacterium tuberculosis and its drug resistance to rifampin and isoniazid. It has significant advantages in the rapid diagnosis of multidrug-resistant tuberculosis .
