目的:探讨细胞角蛋白19(CK19)、β-半乳糖苷结合蛋白(Galectin-3)、间皮表面微绒毛抗原(MC)及糖基化血红素蛋白(TPO)在甲状腺微小乳头状癌和良性乳头状增生中的表达及其在诊断与鉴别诊断中的意义。方法:采用免疫组化SP法检测34例甲状腺微小乳头状癌与31例良性乳头状增生中CK19、Galectin-3、MC和TPO的表达。结果:CK19、Galectin-3、MC和TPO的阳性表达分别为97.1%,97.1%,94.1%,11.8%;在31例甲状腺良性乳头状增生性病变中CK19、Galectin-3、MC和TPO的阳性表达分别为25.8%、22.6%、3.2%、100.0%。CK19、Galectin-3、MC和TPO在甲状腺微小乳头状癌和良性乳头状增生性病变中的表达差异有显著性(P<0.01)。四种抗体在鉴别诊断中的敏感性分别为97.1%、97.1%、94.1%、88.2%;特异性分别为74.2%、77.4%、96.8%、100.0%;准确度分别为86.2%、87.7%、95.4%、93.8%。结论:CK19、Galectin-3、MC及TPO是诊断甲状腺微小乳头状癌的重要标志。联合检测四种抗体在甲状腺微小乳头状癌与甲状腺良性乳头状增生的鉴别诊断中具有重要价值。
Objective: To investigate the effect of cytokeratin 19 (CK19), β-galactoside binding protein (Galectin-3), mesothelial microvilli antigen (MC) and glycosylated hemoprotein (TPO) Expression of benign papillary hyperplasia and its significance in diagnosis and differential diagnosis. Methods: The expressions of CK19, Galectin-3, MC and TPO in 34 cases of thyroid papillary carcinoma and 31 cases of papillary hyperplasia were detected by immunohistochemical SP method. Results: The positive expression rates of CK19, Galectin-3, MC and TPO were 97.1%, 97.1%, 94.1% and 11.8% respectively. The positive rates of CK19, Galectin-3, MC and TPO in 31 cases of benign papillary hyperplasia of thyroid The expression was 25.8%, 22.6%, 3.2%, 100.0% respectively. The expression of CK19, Galectin-3, MC and TPO in thyroid papillary carcinoma and benign papillary proliferative lesions were significantly different (P <0.01). The sensitivity of the four antibodies in differential diagnosis were 97.1%, 97.1%, 94.1% and 88.2% respectively; the specificity was 74.2%, 77.4%, 96.8% and 100.0% respectively; the accuracy was 86.2% and 87.7% 95.4%, 93.8%. Conclusion: CK19, Galectin-3, MC and TPO are important markers in the diagnosis of papillary thyroid carcinoma. Joint detection of four antibodies in the thyroid papillary thyroid carcinoma and benign papillary hyperplasia in the differential diagnosis of great value.
