目的评价我院住院手术患者抗菌药物应用情况及其合理性。方法采用回顾性队列研究,在2006-2008年内按《抗菌药物临床应用指导原则实施细则》出台前后半年及我院实施合理用药机制后半年分成3个阶段,每个阶段随机抽取500例住院患者,对其手术患者病历进行收集分析,评价临床抗菌药物使用的合理性。结果与第一阶段相比,第二阶段的抗菌药物使用金额有所下降,用药天数明显缩短,在预防用药给药疗程和时间、联合用药指征、品种上更为合理;与第二阶段相比,第三阶段用药种类数有所下降,在预防用药类别、疗程和时间、细菌培养和药敏试验、根据药敏结果调整上更加合理。结论《抗菌药物临床应用指导原则实施细则》的出台一定程度降低了抗菌药物在临床的使用量,减少了患者的住院天数,并减少了不合理用药事件的发生,节约了医疗资源。同时通过药师参与临床及各种机制的建立,能够明显加强政策的落实,促进抗菌药物的进一步合理应用。
Objective To evaluate the application of antibacterials in hospitalized patients in our hospital and its rationality. Methods A retrospective cohort study was conducted in 2006-2008. According to the Implementation Rules for Clinical Application of Antibacterials in the first half of 2006 and the second half of the year after the introduction of rational drug use mechanism in our hospital, we divided into three stages in the first half of this year. 500 inpatients, Surgical patients’ medical records were collected and analyzed to evaluate the rationality of clinical antibacterials. Results Compared with the first phase, the amount of antibacterials used in the second phase decreased and the number of days of medication was significantly shortened, which was more reasonable in the course and time of preventive drug delivery, indications and varieties of combination therapy. Compared with the second phase Compared with the third phase, the number of medication types decreased, and it is more reasonable to adjust the drug sensitivity results in the category of preventive medication, duration of treatment, bacterial culture and drug susceptibility testing. Conclusion The introduction of the Implementing Rules of Guiding Principles for Clinical Application of Antibacterial Drugs has reduced the clinical use of antibacterial drugs to a certain extent, reduced the hospitalization days of patients and reduced the occurrence of irrational drug use incidents, saving medical resources. At the same time, participation of pharmacists in the establishment of clinical and various mechanisms can significantly strengthen the implementation of policies and promote the further rational application of antimicrobial agents.
