目的了解大连地区女性生殖道人类乳头瘤病毒(Human Papillomavirus,HPV)感染的感染率、亚型分布及年龄分布情况,为HPV感染的筛查和防治提供科学指导。方法采用聚合酶链式反应(Polymerase Chain Reaction,PCR)体外扩增和DNA反向点杂交相结合的DNA芯片技术对大连地区2010年8月-2012年9月来某院就诊的2 755例成年女性的HPV感染情况进行定性检测并加以分型。结果在2 755例成年女性中,共检出21种HPV基因型,HPV阳性检出率为23.19%。其中,单纯高危型感染率为16.33%,单纯低危型感染率为3.88%,低危型与高危型混合感染率为2.98%;单一、双重、三重、四重型别感染率分别为16.70%、4.61%、1.31%和0.58%。HPV感染率中最高的高危亚型和低危亚型分别为HPV16型(5.81%)和HPV43型(4.03%)。不同年龄检查者的HPV感染率差异和其单一及多重感染构成具有统计学意义,各年龄HPV感染者均以单一及高危感染为主,但在30岁以下及60岁以上年龄HPV感染者中高低危合并多重感染高于其他年龄段。结论大连地区2 755例成年女性共检出21种HPV基因型,HPV阳性检出率为23.19%。检出的HPV感染主要为单一基因型和单纯高危基因型,高危型和低危型则分别以HPV16和HPV43检出率最高。对于不同年龄段的检查者,其HPV感染率不同,单一及多重感染的构成也有差别;同时,30岁以下及60岁以上年龄段女性的高低危合并的多重感染高于其他年龄段的女性。
Objective To understand the infection, subtypes and age distribution of human papillomavirus (HPV) infection in female genital tract in Dalian, and to provide scientific guidance for the screening and prevention of HPV infection. Methods A total of 2 755 adult adolescents from a hospital in Dalian from August 2010 to September 2012 were analyzed by polymerase chain reaction (PCR) in vitro and DNA reverse dot blot hybridization. Female HPV infection was qualitatively tested and classified. Results Among 2 755 adult women, 21 HPV genotypes were detected, and the positive rate of HPV was 23.19%. Among them, the rate of simple high-risk infection was 16.33%, that of simple low-risk infection was 3.88%, that of low-risk and high-risk was 2.98%, that of single, double, triple and quadruple type was 16.70% 4.61%, 1.31% and 0.58% respectively. The highest high-risk and low-risk subtypes of HPV infection were HPV16 (5.81%) and HPV43 (4.03%), respectively. HPV infection rates of different age examiners and their single and multiple infections constitute a statistically significant age-specific HPV infection were single and high-risk infection, but below the age of 30 and HPV infection in patients over the age of 60 were high and low Risk of multiple combined infection is higher than other age groups. Conclusions A total of 2175 HPV genotypes were detected in 2 755 adult women in Dalian. The positive rate of HPV was 23.19%. HPV infection was detected mainly single genotype and high-risk genotypes, high-risk type and low-risk type were the highest detection rate of HPV16 and HPV43 respectively. For people of different ages, HPV infection rates are different, single and multiple infections are also different in composition; the same time, high and low risk of multiple infection in women aged 30 and below and women over the age of 60 were higher than other age groups of women.
