目的 评价、比较弹性开放襻前房型与巩膜缝线固定襻后房型人工晶体的疗效及安全性。方法 回顾性研究 6 8例 (6 8只眼 )后囊缺失无晶体眼的人工晶体植入术的手术结果。根据植入人工晶体的类型分为两组。第一组 (30只眼 )为施行弹性开放襻前房型人工晶体植入术者。第二组 (38只眼 )为施行巩膜缝线固定襻后房型人工晶体植入术者。使用超声生物显微镜 ,结合前房角镜 ,观察术后人工晶体襻的准确位置 ,襻与周围组织的关系 ,以评价这两种人工晶体对眼前段结构的影响。随访 6个月至 2 0个月。结果 术后矫正视力达 0 5以上者 ,第一组有 2 7只眼 (占 90 0 %) ,第二组有 35只眼 (占 92 1%)。两组比较无统计学意义 (P >0 0 5 )。在第一组有 2 3只眼 (76 7%)出现手术并发症 ,总并发症数为 4 0次 /眼 ;而在第二组分别为13只眼 (34 2 %)和 19次 /眼。两组比较有统计学意义 (P <0 0 5 )。超声生物显微镜和前房角镜检查显示 :在第一组 ,所有的人工晶体襻均与虹膜完全接触 ,且不同程度压迫虹膜 ,甚至引起房角后退。虹膜前粘连 12只眼。在6 0个人工晶体襻中 ,39个襻的固定脚板固定于睫状体带 ,其余 2 1个襻 (12只眼 )穿过虹膜侵入到睫状体实质内 ,伴有反复发作的葡萄膜炎。在第二组的 76个人工晶体襻中 ,5
Objective To evaluate and compare the efficacy and safety of anterior chamber type with scleral suture fixation and Posterior chamber intraocular lens (IOL). Methods The retrospective study of 68 cases (68 eyes) posterior capsule loss of aphakia intraocular lens implantation surgery results. According to the type of implanted intraocular lens is divided into two groups. The first group (30 eyes) for the implementation of elastic open 襻 anterior chamber intraocular lens implantation. The second group (38 eyes) was performed with scleral suture fixation and posterior chamber intraocular lens implantation. Using ultrasound biomicroscopy combined with gonioscopy, observe the exact location of the posterior intraocular lens, and the relationship with the surrounding tissue to evaluate the effect of the two intraocular lenses on the anterior segment of the anterior segment. Follow up from 6 months to 20 months. Results There were 27 eyes (90 0%) in the first group and 35 eyes (92 1%) in the second group. There was no significant difference between the two groups (P> 0.05). There were 23 (76.7%) surgical complications in the first group, with a total complication of 40 beats / eye; in the second group, 13 (34.2%) and 19 .
