目的:探讨改进式SBAR交班模式在胸科ICU应用的效果。方法根据呼吸内科患者的特点,改进 SBAR 床边交班模式,比较改进前后的效果。结果①患者压疮和非计划拔管率分别有1.18%和1.02%降至0.31%和0.36%;②患者满意度提高,交接班时间延长;③护士对患者整体情况的知晓率(包括病情及治疗、护理问题、护理风险、护理方案)(P <0.05)和护理措施率提高(包括生活护理、健康教育、监护护理、治疗执行)(P <0.05)。结论改进式SBAR床边交班模式有利降低护理风险,提高护理质量,有利管理者对护士的考核,也有利于低年资护士的成长,值得推广。“,”Objective To explore the application effect of SBAR shift model in the ICU . Methods According to the characteristics of the ICU,to improve SBAR shift model, and to compare the effect of before and after . Results ①the pressure ulcers rate and unplanned extubation rate are from 1.18% and 1.02% to 1.18%and 1.18% respectively; ②the patient satisfaction, extended their succession; ③the nurses konw patients in the overall population (include the illness and treatment,nursingproblems,nursing risk and plan)(P< 0.05) and the nursing measures rate increased(include life nursing,healtheducation,critical care and treatment) (P< 0.05). Conclusion The SBAR shift model was beneficial to reduce nursing risks, to improve the quality of nursing, to appraise nurses , also conducive to the growth of low qualification nurses. So it is worth promoting.
