急性白血病要获完全缓解 (CR)必须经过化疗后的骨髓增生抑制期。在此期间外周血白细计数必然急剧减少。为了观察急性白血病化疗后白细胞 (WBC)计数与化疗疗效之间的关系 ,对 80例初治急性白血病患者在诱导化疗第 1疗程后WBC计数与疗效关系进行了分析。根据化疗后白细胞数的最低值分为 3组 :≤ 0 .4× 10 9/L组 ;(0 .4 - 0 .9)× 10 9/L组 ;>0 .9× 10 9/L组。结果表明 :初治急性白血病化疗第 1疗程后 ,WBC计数≤ 0 .4× 10 9/L的急性白血病CR率 6 0 % ,总有效率 90 % ;WBC计数在 (0 .4 - 0 .9)× 10 9/L的急性白血病CR率 5 5 .6 % ,总有效率 92 .6 % ;WBC计数 >0 .9× 10 9/L的急性白血病CR率 2 7.3% ,总有效率 6 6 .7%。白细胞≤ 0 .4× 10 9/L组 ,(0 .4 - 0 .9)× 10 9/L组与 >0 .9× 10 9/L组第 1疗程化疗有效率有显著性差异 (P <0 .0 1)。而白细胞≤ 0 .4× 10 9/L组与白细胞 (0 .4 - 0 .9)× 10 9/L组无明显差异。结论 :初发急性白血病第 1疗程化疗后白细胞计数可作为急性白血病化疗的一个预测指标。
Acute leukemia to be completely relieved (CR) must be post-chemotherapy bone marrow suppression period. During this period the white blood cell count inevitably sharply reduced. In order to observe the relationship between leucocyte (WBC) count and chemotherapy efficacy after acute leukemia chemotherapy, the relationship between WBC count and curative effect was analyzed in 80 patients with newly diagnosed acute leukemia after the first course of induction chemotherapy. According to the lowest number of leukocytes after chemotherapy, they were divided into 3 groups: ≤0.4 × 10 9 / L group, (0.4-0.9) × 10 9 / L group,> 0.9 × 10 9 / L group . The results showed that the CR rate of acute leukemia with WBC count ≤0.4 × 10 9 / L was 60% and the total effective rate was 90% after the first course of chemotherapy-naive acute leukemia chemotherapy. The WBC count was between (0.4-0.9 ) Acute lymphoblastic leukemia with the rate of 109 / L was 5.56%, the total effective rate was 92.6%; the CR rate of acute leukemia with WBC count> 0.9 × 10 9 / L was 7.33% and the total effective rate was 6 6 .7%. The effective rate of chemotherapy in the first course of leukocyte ≤0.4 × 10 9 / L group, (0.4-0.9) × 10 9 / L group and> 0.9 × 10 9 / L group was significantly different (P <0 .0 1). However, there was no significant difference between white blood cells ≤ 0. 4 × 10 9 / L group and white blood cells (0. 4 - 0. 9) × 10 9 / L group. Conclusion: The first course of acute leukemia after chemotherapy leukocyte count can be used as a predictor of acute leukemia chemotherapy.
