目的观察脑出血大鼠脑组织中基质金属蛋白酶-2(MMP-2)表达,探讨补阳还五汤治疗脑出血的作用机制。方法采用立体定向技术制作大鼠脑出血模型,对出血后2、4、7、14、21、28 d后各组脑组织进行MMP-2免疫组织化学染色,检测脑组织各区域MMP-2的阳性微血管数。结果正常组未见表达,假手术组皮质偶见MMP-2表达;模型组大鼠MMP-2呈双峰表达,在造模后4 d达最高峰(P<0.01),之后有所下降,至21 d仍有表达并再有小高峰出现(P<0.01);补阳还五汤组4 d时为表达低谷,并明显低于模型组(P<0.01),其后逐渐上升至高水平维持达2 w,其中7、14、21 d表达均高于模型组(P<0.05),28 d表达稍下降但仍高于模型组(P<0.01)。结论补阳还五汤可通过调节脑出血后MMP-2的表达,有利于微血管系统重建,改善出血后脑组织的微环境。
Objective To observe the expression of matrix metalloproteinase-2 (MMP-2) in the brain of rats with intracerebral hemorrhage and explore the mechanism of Buyang Huanwu Decoction in treatment of cerebral hemorrhage. Methods A rat model of intracerebral hemorrhage was made using stereotactic technique. MMP-2 immunohistochemical staining was performed on 2, 4, 7, 14, 21, and 28 days after hemorrhage, and MMP-2 was detected in various regions of brain tissue. The number of positive microvessels. Results No expression was found in the normal group. Expression of MMP-2 was observed occasionally in the sham-operated group. MMP-2 expression was double-peak in the model group, peaked at 4 days after modeling (P<0.01), and decreased afterwards. At the 21st day, there was still expression and there was a small peak (P<0.01); the Buyang Huanwu Decoction group had a trough on the 4th day and was significantly lower than the model group (P<0.01), and then gradually increased to a high level. At 2 w, the expressions on 7, 14 and 21 d were higher than those in the model group (P<0.05). The expression on 28 d was slightly decreased but still higher than that in the model group (P<0.01). Conclusion Buyang Huanwu decoction can be used to regulate the expression of MMP-2 after intracerebral hemorrhage, which is beneficial to the reconstruction of microvascular system and improve the microenvironment of brain tissue after hemorrhage.
