目的探讨白细胞介素18(IL-18)-607A/C及单核细胞趋化蛋白-1(MCP-1)-2518A/G基因多态性与类风湿关节炎(RA)易感性的关系。方法制定检索策略及纳入标准,计算机检索PubMed、Web of knowledge、万方数据库、中国生物医学文献数据库及中国学术期刊全文数据库自1990年1月至2013年9月国内外已发表有关IL-18-607A/C及MCP-1-2518A/G基因多态性与RA易感性的病例对照研究,采用RevMan 5.0软件对各文献进行异质性检验和Meta分析,得到合并后的OR值及其95%CI。结果共16篇文献纳入Meta分析,IL-18-607A/C文献12篇(RA组3 527例,对照组3 161例),MCP-1-2518A/G文献4篇(RA组1 460例,对照组1 303例)。Meta分析结果显示,IL-18-607基因型CC与RA有关联(OR=1.27,95%CI=1.02~1.58),排除不符合Hardy-Weinberg平衡(HWE)的文献,按人种分层发现亚裔人群中,IL-18-607等位基因C与RA易感性有关联(OR=1.33,95%CI=1.01~1.76)。未发现MCP-1-2518A/G多态性与RA易感性有关联。结论 IL-18-607A/C位点突变是RA易感性的一个危险因素。MCP-1-2518位点多态性与RA易感性是否有关联尚无定论。
Objective To investigate the relationship between polymorphisms of interleukin 18 (IL-18) -607A / C and MCP-1 -2518A / G and susceptibility to rheumatoid arthritis (RA). Methods To establish a search strategy and inclusion criteria for the computer search of PubMed, Web of knowledge, Wanfang database, China Biomedical Literature Database and Chinese Academic Journal Full-text Database Since 1990 January to September 2013 at home and abroad have published about IL-18- 607A / C and MCP-1-2518A / G polymorphisms and susceptibility to RA case-control study, the use of RevMan 5.0 software heterogeneity of the literature and Meta analysis, the combined OR value and 95% CI. Results A total of 16 articles were included in the meta-analysis. There were 12 articles of IL-18-607A / C (3 527 cases in RA group and 3 161 cases in control group), 4 articles in MCP-1-2518A / G (1 460 cases in RA group, Control group 1 303 cases). Meta-analysis showed that genotype CC of IL-18-607 was associated with RA (OR = 1.27, 95% CI = 1.02-1.58), excluding those who did not meet the Hardy-Weinberg equilibrium (HWE) In the Asian population, IL-18-607 allele C was associated with RA susceptibility (OR = 1.33, 95% CI = 1.01-1.76). No MCP-1-2518A / G polymorphisms were found to be associated with RA susceptibility. Conclusion IL-18-607A / C locus mutation is a risk factor for RA susceptibility. Whether MCP-1-2518 polymorphism is associated with RA susceptibility remains to be determined.
