目的:探讨对比增强超声(CEUS)在鉴别慢性肾病(CKD)患者的肾脏恶性肿瘤病变中的敏感度、特异性及精确度。方法:选取2018年1月至2019年1月本院收治的106例肾脏肿瘤患者,均进行CEUS影像学检查,由2名超声科医师按照CEUS标准进行成像作业。将所有患者的CEUS影像结果同组织学进行比较,分析其敏感度、特异性和精确度。结果:在所有患者中,CEUS检测恶性肿瘤的敏感度为95.5%(95%n CI: 84.0~100.0),特异性为50.3%(95%n CI: 33.1~70.0)。n 结论:CEUS对诊断CKD患者的肾脏恶性肿瘤病变具有较高的敏感度,但特异性较低,修正对比增强超声的分类标准可提高特异性。而其敏感度的优异性,使得CEUS对CKD患者排查恶性肿瘤具有较高的临床诊断价值。“,”Objective:To evaluate the sensitivity, specificity, and accuracy of contrast-enhanced ultrasound(CEUS) for identifying malignancy of kidney lesions in patients with chronic kidney disease(CKD).Methods:We performed CEUS on 106 patients.Two radiologists categorized lesions using CEUS images according to contrast-enhanced ultrasound adapted criteria. CEUS designation was compared to histology to determine sensitivity, specificity and overall accuracy.Results:Across all patients, CEUS had a sensitivity of 95.5%(95%n CI: 84.0-100.0) and specificity of 50.3%(95%n CI: 33.1-70.0) for detecting malignancy.n Conclusions:CEUS has high sensitivity for identifying malignancy of kidney lesions, but low specificity.The classification scheme for contrast-enhanced ultrasound were modified to improve the specificity of contrast-enhanced ultrasound.Due to its sensitivity, among patients with chronic kidney disease or other contrast contraindications, CEUS still has potential diagnostic value as an imaging test to rule out malignancy.
