目的:研究鼻咽癌多层螺旋CT各灌注参数的改变和良恶性肿瘤的鉴别诊断。方法:30例鼻咽癌患者行鼻咽血流的CT灌注扫描,30例鼻咽癌患者中有25例行放疗后复查,其中复查时行灌注扫描6例。应用去卷积算法模式计算相应病变区域的血流量(BF)、血容量(BV)、平均通过时间(MTT)和毛细血管通透性(PS)。根据不同肿瘤的相关参数图来评价鼻咽癌血流动力学状态。结果:30例鼻咽癌患者BF、BV和PS明显高于肿瘤放疗后组织及正常鼻咽组织,而MTT值差异无统计学意义。结论:灌注参数BF、BV和PS可有效地评价鼻咽癌的血流状态,在鼻咽良恶性病变的鉴别诊断及预后的判断上有重要的临床参考价值。
Objective: To study the changes of perfusion parameters and the differential diagnosis of benign and malignant tumors in multi-slice spiral CT of nasopharyngeal carcinoma. Methods: Thirty patients with nasopharyngeal carcinoma underwent CT perfusion scan of nasopharyngeal blood flow. Among 30 patients with nasopharyngeal carcinoma, 25 patients were examined after radiotherapy. Among them, 6 patients underwent perfusion scan at the time of review. The BF, BV, MTT, and PS of the corresponding lesion area were calculated using the deconvolution algorithm. The hemodynamic status of nasopharyngeal carcinoma was evaluated according to the relevant parameters of different tumors. Results: The BF, BV and PS in 30 patients with nasopharyngeal carcinoma were significantly higher than those after radiotherapy and the normal nasopharyngeal tissues, while the MTT values showed no significant difference. Conclusion: Perfusion parameters BF, BV and PS can effectively evaluate the blood flow status of nasopharyngeal carcinoma. It has important clinical reference value in the differential diagnosis and prognosis judgment of benign and malignant nasopharyngeal lesions.
