目的:本文探讨双U形黏膜瓣修复加局部扩张术治疗鼻咽闭锁的方法和效果。方法:选取9例后天性鼻咽闭锁患者,采用双U形黏膜瓣修复咽后壁和软腭创面,再置入扩张管。结果:6个月后取出硅胶管,鼻咽腔缩小到左右径约为2.0～2.5cm,前后径约为1.0cm的间隙。所有患者鼻腔通畅,但均有腭咽闭合不全、食物鼻腔反流。术后2年随访,患者鼻腔均通畅,睡眠、饮食均正常。1例仍有食物鼻腔反流。结论:U形黏膜瓣修复加局部扩张术是治疗鼻咽闭锁行之有效的方法。
Objective: This article discusses the double U-shaped mucosa flap repair and local expansion of the treatment of nasopharyngeal atresia methods and results. Methods: Nine patients with acquired nasopharyngeal atresia were selected. Double U-shaped mucosal flap was used to repair the posterior pharyngeal wall and the soft palate wound and then placed into dilation tube. Results: After 6 months, the silicone tube was removed. The nasopharynx was narrowed to a gap of about 1.0 ~ 2.5 cm with a diameter of about 2.0 ~ 2.5 cm. Nasal patency in all patients, but have velopharyngeal insufficiency, nasal reflux food. After 2 years of follow-up, the patients had smooth nasal passages, normal sleep and normal diet. One patient still had food nasal reflux. Conclusion: U-shaped mucosal flap repair plus local dilation is an effective method for the treatment of nasopharyngeal atresia.
