目的研究原发性高血压病患者血清胰岛素样生长因子-1(IGF-1)和C反应蛋白(CRP)水平与颈动脉内膜中层厚度(IMT)的相关性。方法高血压病患者65例,健康查体者(正常对照组)60例。用放射免疫法检测血清IGF-1水平,免疫散射比浊法测定血清中CRP含量,高分辨超声测定IMT作为大血管动脉粥样硬化(AS)程度的指标,分析IGF-1、CRP与IMT之间的关系。结果与正常对照组相比,EH组的血清IGF-1、CRP水平均显著增高(P<0.01),IMT显著增厚(P<0.01)。随着高血压分级水平的增高,IMT逐步增厚,颈动脉粥样硬化的程度逐渐加重,IGF-1和CRP浓度也增大。相关分析显示,血清IGF-1、CRP与IMT呈正相关(r=0.227,0.318;P<0.05,P<0.05)。结论血清IGF-1、CRP水平与EH患者IMT正相关,是EH患者IMT增厚的危险因素,提示IGF-1、CRP参与了EH患者AS病理过程,且其水平的高低与AS严重程度有关,可作为EH患者并发AS的早期预测及监测指标。
Objective To investigate the relationship between serum levels of insulin-like growth factor-1 (IGF-1) and C-reactive protein (CRP) and carotid artery intima-media thickness (IMT) in patients with essential hypertension. Methods Totally 65 hypertensive patients and 60 healthy volunteers (normal control group). Serum IGF-1 levels were measured by radioimmunoassay, serum CRP levels by immunostaining turbidimetric assay, and IMT as a marker of macrovascular atherosclerosis (AS) by high resolution ultrasound. Results Compared with the normal control group, the levels of serum IGF-1 and CRP in EH group were significantly increased (P <0.01) and IMT was significantly increased (P <0.01). As the level of hypertension increased, IMT gradually increased, the degree of carotid atherosclerosis gradually increased, IGF-1 and CRP concentrations also increased. Correlation analysis showed that there was a positive correlation between serum IGF-1, CRP and IMT (r = 0.227,0.318; P <0.05, P <0.05). Conclusions Serum levels of IGF-1 and CRP are positively correlated with IMT in EH patients, which may be the risk factors of IMT thickening in EH patients. It suggests that IGF-1 and CRP are involved in the pathological process of AS in EH patients, and their levels are related to the severity of AS. EH patients can be used as an early prediction of AS and monitoring indicators.
