目的:探讨CC趋化因子受体7(CCR7)和L-选择素(L-selectin)在肺腺癌组织中的表达及意义。方法:免疫组织化学法检测40例肺腺癌和20例癌旁正常肺组织中CCR7和L-选择素的表达。结果:40例肺腺癌组织中CCR7阳性表达37例(92.5%),L-选择素阳性表达27例(67.5%);20例癌旁正常肺组织中CCR7阳性表达3例(15.0%),L-选择素阳性表达0例。肺腺癌组织CCR7和L-选择素阳性率明显高于癌旁正常肺组织,χ2值分别为36.04、24.55,P值均为0.0000;并且CCR7表达与L-选择素表达有显著正相关性,χ2=4.84,P=0.0288。24例伴淋巴结转移的病例CCR7和L-选择素阳性率(分别为100.0%和83.3%)明显高于16例不伴淋巴结转移的病例(分别为81.3%和43.8%),χ2值分别为4.86、6.86,P值分别为0.0284、0.0091。Ⅰ、Ⅱ期肺腺癌病例中CCR7和L-选择素阳性率分别为83.3%(15/18)和50.0%(9/18);Ⅲ、Ⅳ期肺腺癌病例中CCR7和L-选择素阳性率分别为100.0%(22/22)和81.8%(18/22)。CCR7和L-选择素表达均与TNM分期存在明显正相关性,χ2值分别为3.96和4.56,P值分别为0.0470、0.0348。结论:肺腺癌组织中CCR7与L-选择素表达具有显著正相关性,两者可能共同参与了肺腺癌的发生及淋巴结转移过程,检测两者表达情况有利于判断肺腺癌患者的TNM分期和预后。
Objective: To investigate the expression of CCR7 and L-selectin in lung adenocarcinoma and its significance. Methods: Immunohistochemistry was used to detect the expression of CCR7 and L-selectin in 40 cases of lung adenocarcinoma and 20 cases of adjacent normal lung tissues. Results: The positive expression of CCR7 in 40 cases of lung adenocarcinoma was 37 (92.5%) and the positive expression of L-selectin was 27 cases (67.5%). The positive expression of CCR7 in 20 cases of normal lung tissue was 3 cases (15.0%), 0 cases of positive expression of L-selectin. The positive rates of CCR7 and L-selectin in lung adenocarcinoma tissues were significantly higher than those in adjacent normal lung tissues (χ2 = 36.04,24.55, P = 0.0000 respectively), and there was a significant positive correlation between CCR7 expression and L-selectin expression χ2 = 4.84, P = 0.0288. The positive rates of CCR7 and L-selectin in the cases with lymph node metastasis (100.0% and 83.3% respectively) were significantly higher than those in 16 cases without lymph node metastasis (81.3% and 43.8 %), χ2 values were 4.86,6.86, P values were 0.0284,0.0091. The positive rates of CCR7 and L-selectin in stage I and II lung adenocarcinomas were 83.3% (15/18) and 50.0% (9/18) respectively. In stage III and IV lung adenocarcinoma, CCR7 and L-selectin The positive rates were 100.0% (22/22) and 81.8% (18/22), respectively. The expressions of CCR7 and L-selectin were positively correlated with TNM staging. The χ2 values were 3.96 and 4.56 respectively, and the P values were 0.0470 and 0.0348 respectively. Conclusion: There is a significant positive correlation between the expression of CCR7 and L-selectin in lung adenocarcinoma. Both of them may participate in the occurrence of lung adenocarcinoma and the process of lymph node metastasis. To detect the expression of CCR7 and L-selectin is helpful to judge the TNM Staging and prognosis.
