目的探讨自体回输血技术常温下全肝血流阻断治疗常规手术方法不能切除的肝巨大血管瘤。方法采用国产自体血液回输机,对8例贴近多个肝门的肝巨大血管瘤常规在肝上下腔静脉、肝下下腔静脉、第一肝门放置阻断带,根据术中需要行不同时间的全肝血流阻断修补损伤血管。结果全组8例全部痊愈出院,除1例游离肝脏时瘤体破裂未能及时行全肝血流阻断致大出血术中输血6000毫升外,其余7例术中均未输血。结论采用自体回输血常温下全肝血流阻断技术切除常规手术方法不能切除的肝巨大血管瘤,是一种切实可行的办法。
Objective To investigate the treatment of hepatic giant hemangiomas which can not be removed by conventional surgical methods with autologous transfusion of blood at room temperature. Methods 8 patients with giant hepatic hemangiomas close to multiple hepatic vessels were placed in the superior and inferior vena cava, inferior hepatic vein and first hepatic portal by using a homemade autologous blood transfusion machine. According to the needs of the operation, The time of the whole liver blood flow blocked the repair of damaged blood vessels. Results All 8 cases were cured and discharged. Except one case of rupture of the free liver, blood transfusion of 6,000 ml was prevented during the operation. The other 7 cases had no intraoperative blood transfusion. Conclusion The autologous transfusion of whole liver blood flow blocking technique at normal temperature resection of unresectable hepatic giant hemangioma is a feasible way.
