目的:探讨乳腺癌患者血浆胆固醇27-羟化酶(CYP27A1)和胆固醇7α羟化酶(CYP7B1)浓度与乳腺癌临床分期、组织学分级、Ki-67等预后因素的相关性。方法:观察分析了34例乳腺癌患者血浆CYP27A1和CYP7B1浓度,及其与血清总胆固醇、乳腺癌临床分期、组织学分级、Ki-67等指标的关系。结果:34例乳腺癌患者中,总胆固醇升高者16例,总胆固醇水平正常者18例,血清总胆固醇升高的乳腺癌患者,其血浆CYP27A1酶浓度较总胆固醇正常患者高(P<0.05),而CYP7B1酶浓度明显低于总胆固醇正常患者(P<0.01)。Ki-67高表达乳腺癌患者血中27HC合成酶CYP27A1水平较Ki-67低表达者高(P<0.05);而27HC降解酶CYP7B1水平与Ki-67水平的关系则正好相反。临床Ⅰ期乳腺癌患者血浆CYP27A1浓度明显低于Ⅱ、Ⅲ、Ⅳ期,与之相反,CYP7B1浓度却高于Ⅱ、Ⅲ、Ⅳ期患者,结果均有显著性差异(P<0.05);Ⅱ、Ⅲ、Ⅳ期乳腺癌患者间血浆CYP27A1、CYP7B1浓度无显著性差异(P>0.05)。组织学分级间亦有类似的结果。结论:Ki-67低表达、分化好的早期乳腺癌患者,其血浆CYP27A1水平偏低,CYP7B1水平较高,27HC代谢酶与乳腺癌预后因素具有一定的相关。
Objective: To investigate the correlation between plasma cholesterol 27-hydroxylase (CYP27A1) and cholesterol 7α-hydroxylase (CYP7B1) levels in breast cancer patients and the clinical stage, histological grade, Ki-67 and other prognostic factors in patients with breast cancer. Methods: The plasma concentrations of CYP27A1 and CYP7B1 in 34 patients with breast cancer were analyzed. Their relationships with serum total cholesterol, clinical stage, histological grade and Ki-67 were also analyzed. Results: Of the 34 patients with breast cancer, 16 patients with elevated total cholesterol and 18 patients with normal total cholesterol had higher serum CYP27A1 concentrations than patients with normal total cholesterol (P <0.05) ), While the concentration of CYP7B1 enzyme was significantly lower than that of patients with normal total cholesterol (P <0.01). The level of 27HC synthase CYP27A1 in Ki-67 overexpressing breast cancer patients was significantly higher than that in Ki-67 overexpression (P <0.05), while the relationship between 27HC degrading enzymes CYP7B1 and Ki-67 was the opposite. The plasma level of CYP27A1 in patients with clinical stage Ⅰ breast cancer was significantly lower than that in stage Ⅱ, Ⅲ and Ⅳ. On the contrary, the concentration of CYP7B1 was higher than that in stage Ⅱ, Ⅲ and Ⅳ (P <0.05) There was no significant difference in plasma CYP27A1 and CYP7B1 levels between Ⅲ and Ⅳ breast cancer patients (P> 0.05). Histological grade also had similar results. CONCLUSIONS: In patients with low-expression and well-differentiated Ki-67, the levels of plasma CYP27A1 and CYP7B1 are higher in patients with early-stage breast cancer. The relationship between 27HC-metabolizing enzymes and the prognosis of breast cancer is also related.
