目的探讨中国汉族人群肺结核发病与CD209基因-336位点多态性的关系,并分析其在肺结核发病中与环境因素的交互作用。方法采用1∶1配对病列对照设计,用序列特异性引物-聚合酶链反应(SSP-PCR)技术检测167对病例和对照的CD209基因-336位点的基因型分布;对肺结核相关环境因素进行问卷调查,并进行单因素和多因素条件Logistic回归分析。结果AA、GA和GG基因型在病例组和对照组的分布频率分别为21.0%,42.5%,36.5%和35.3%,45.4%,19.1%。单因素条件Logistic回归分析显示,GG基因型在病例组中频率显著高于对照组(OR=1.768,OR95%CI=1.167~2.679,P=0.007)。调整环境因素后,多因素条件Logistic回归分析显示,GG基因型与肺结核发病仍有关联(OR=1.918,OR95%CI=1.234~2.979,P=0.004);GG和GA基因型与卡介苗接种史(P=0.000)以及与吸烟史(P=0.000)对肺结核发病均有正相乘模型交互作用。结论CD209基因-336位点GG基因型可能是中国汉族人的肺结核易感基因型,GG和GA基因型可能与环境因素存在交互作用。
Objective To investigate the relationship between the incidence of pulmonary tuberculosis and the polymorphism of -336 locus in CD209 gene of Chinese Han population and to analyze its interaction with environmental factors in the pathogenesis of pulmonary tuberculosis. Methods A 1: 1 matched control group was used to design the genotype distribution of -336 locus of CD209 gene in 167 cases and controls by sequence-specific primer-polymerase chain reaction (SSP-PCR) Conducted questionnaires, and single-factor and multi-factor conditional Logistic regression analysis. Results The distribution frequencies of AA, GA and GG genotypes were 21.0%, 42.5%, 36.5% and 35.3%, 45.4% and 19.1% in case group and control group respectively. Logistic regression analysis showed that the genotype of GG in the case group was significantly higher than that in the control group (OR = 1.768, OR 95% CI = 1.167-2.667, P = 0.007). Logistic regression analysis showed that the GG genotype was still associated with the incidence of pulmonary tuberculosis (OR = 1.918, OR 95% CI = 1.234-2.979, P = 0.004) after adjustment for environmental factors. The genotypes of GG and GA were significantly different from those of BCG vaccination P = 0.000) and smoking history (P = 0.000) had a positive multiplication model interaction on pulmonary tuberculosis. Conclusion GG genotype at -336 locus of CD209 gene may be a susceptible genotype of pulmonary tuberculosis in Han Chinese, while GG and GA genotypes may interact with environmental factors.
