目的探讨血清转化生长因子β1(TGFβ1)、肿瘤坏死因子α(TNFα)与2型糖尿病肾病的关系。方法采用双抗体夹心酶联免疫法和放射免疫分析法测定单纯糖尿病、早期糖尿病肾病、临床糖尿病肾病患者(各30例)空腹TGFβ1、TNFα水平,30例健康献血者作正常对照。结果早期糖尿病肾病及临床糖尿病肾病患者TGFβ1、TNFα水平明显高于单纯糖尿病患者和正常对照(P<0.05或P<0.01),且随病情进展逐渐升高;单纯糖尿病患者与正常对照比较差异无显著性意义(P>0.05)。相关及回归分析显示TGFβ1、TNFα与尿白蛋白排泄率(UAER)呈正相关。多元线性逐步回归分析显示:TGFβ1、TNFα是影响糖尿病肾病的主要危险因素。结论TGFβ1、TNFα在糖尿病肾病的发生、发展中起重要作用。
Objective To investigate the relationship between serum transforming growth factor β1 (TGFβ1), tumor necrosis factor α (TNFα) and type 2 diabetic nephropathy. Methods The levels of fasting serum TGFβ1 and TNFα in patients with simple diabetes mellitus, early diabetic nephropathy and clinical diabetic nephropathy (all 30 cases) were measured by double antibody sandwich enzyme-linked immunosorbent assay and radioimmunoassay. Thirty healthy donors were used as normal controls. Results The levels of TGFβ1 and TNFα in patients with early diabetic nephropathy and clinical diabetic nephropathy were significantly higher than those with simple diabetes and normal controls (P <0.05 or P <0.01), and gradually increased with the progression of the disease. There was no significant difference between the patients with simple diabetes and the normal controls Sexual significance (P> 0.05). Correlation and regression analysis showed that TGFβ1, TNFα and urinary albumin excretion rate (UAER) was positively correlated. Multivariate linear stepwise regression analysis showed that TGFβ1 and TNFα were the main risk factors affecting diabetic nephropathy. Conclusion TGFβ1 and TNFα play an important role in the development and progression of diabetic nephropathy.
