目的:通过免疫组化检测膜细胞骨架结合早期胃癌和正常胃黏膜组织Ezrin和CD34的表达水平,探讨Ezrin和CD34的表达水平与早期胃癌的发生、发展和转移的关系。方法:采用免疫组化方法检测74例早期胃癌患者和36例正常胃黏膜标本中Ezrin和CD34的表达情况并结合临床病理情况进行综合分析。结果:早期胃癌和正常胃黏膜组织中Ezrin的阳性表达率分别为85.13%和8.33%。CD34染色显示,早期胃癌组织中Ezrin阳性表达患者MVD的数值〔(68.4±2.34)mm2〕高于正常胃黏膜组织〔(16.8±2.10)mm2〕,差异有统计学意义,P<0.01;也高于Ezrin阴性MVD数值〔(32.4±1.80)mm2〕,差异有统计学意义,P<0.01。Ezrin的表达与早期胃癌组织的分化程度和淋巴结转移密切相关,P<0.01;与浸润深度相关性不大。Ezrin的表达程度与早期胃癌组织的CD34染MVD呈正相关。结论:早期胃癌组织中Ezrin的高表达与CD34表达显示的MVD与其分化程度,浸润深度和淋巴结转移有关,两者联合检测可能成为早期胃癌预后预测指标,期望能为胃癌的新的基因靶向治疗提供理论依据。
OBJECTIVE: To detect the expression levels of Ezrin and CD34 in membrane cytoskeleton associated with early gastric cancer and normal gastric mucosa by immunohistochemistry and to explore the relationship between the expression of Ezrin and CD34 and the occurrence, development and metastasis of early gastric cancer. Methods: The expressions of Ezrin and CD34 in 74 cases of early gastric cancer and 36 cases of normal gastric mucosa were detected by immunohistochemical method and analyzed comprehensively with clinicopathological factors. Results: The positive rates of Ezrin in early gastric cancer and normal gastric mucosa were 85.13% and 8.33%, respectively. CD34 staining showed that MVD in Ezrin-positive patients with early stage gastric cancer was (68.4 ± 2.34) mm2 higher than that in normal gastric mucosa 〔(16.8 ± 2.10) mm2〕, the difference was statistically significant, P <0.01; The Ezrin negative MVD value 〔(32.4 ± 1.80) mm2〕, the difference was statistically significant, P <0.01. The expression of Ezrin was closely related to the differentiation and lymph node metastasis of early gastric cancer (P <0.01), but not to the depth of invasion. The expression of Ezrin was positively correlated with CD34-MVD in early gastric cancer. Conclusion: The high expression of Ezrin and the expression of CD34 in early gastric cancer are related to the degree of differentiation, depth of invasion and lymph node metastasis. The combined detection of Ezrin and CD34 expression may be the prognostic indicator of early gastric cancer, which is expected to be a new gene-targeted therapy for gastric cancer Provide a theoretical basis.
