目的:观察腺苷注射液对家兔心肌缺血再灌注损伤的保护作用。方法:雄性纯种新西兰白兔随机分成5组:生理盐水组、腺苷低剂量组、腺苷中剂量组、腺苷高剂量组及阳性对照药硝酸异山梨酯组。结扎冠状动脉左前降支使心肌缺血,80 min后恢复血流,并持续灌注40 min,复制缺血再灌注损伤模型,治疗组分别于结扎40 min后经股静脉静滴腺苷114、228、456μg.kg-1.min-1、硝酸异山梨酯3μg.kg-1.min-1,生理理水组给予等体积生理盐水。记录不同时点的肢体Ⅱ导联和体表各导联的心电图,观察各组在不同时间点血压、心率、心电图ST段的变化,测定心肌梗死范围的百分比,评价腺苷对心肌缺血再灌注损伤的治疗作用。结果:腺苷和硝酸异山梨酯对血压和心率无明显影响,但腺苷明显减轻ST-T段的抬高,同时使心肌梗死范围明显缩小,并呈剂量依赖性,药效与硝酸异山梨酯相似。结论:腺苷对心肌缺血再灌注损伤有保护作用。
Objective: To observe the protective effect of adenosine injection on myocardial ischemia-reperfusion injury in rabbits. Methods: Male purebred New Zealand white rabbits were randomly divided into 5 groups: saline group, adenosine low dose group, adenosine middle dose group, adenosine high dose group and positive control drug isosorbide dinitrate group. Ligation of the left anterior descending coronary artery to myocardial ischemia, blood flow was restored after 80 min, and continued perfusion for 40 min, replicating ischemia-reperfusion injury model, the treatment group were intravenous infusion of adenosine 114, 228, 456μg.kg-1.min-1, isosorbide dinitrate 3μg.kg-1.min-1, physiological saline group given an equal volume of normal saline. The electrocardiogram (ECG) of limbs Ⅱ leads and body leads at different time points were recorded. The changes of blood pressure, heart rate and ST segment of electrocardiogram at different time points were observed. The percentage of myocardial infarction area was measured. However, adenosine significantly reduced ST-T elevation and significantly reduced infarct size in a dose-dependent manner. Ester similar.
