目的了解安徽省铜陵地区临床分离的深部真菌分类和耐药状况。方法2006年1~12月铜陵地区临床分离深部真菌,用生物梅里埃试剂盒API20CAUX进行鉴定分类,ATB-FUNGus21NT进行药敏试验。结果白色假丝酵母菌270株占64.3%,热带假丝酵母菌75株占17.8%,克柔假丝酵母菌20株占4.76%,近平滑假丝酵母菌18株占4.3%,光滑假丝酵母菌16株占3.8%,季也蒙假丝酵母菌7株占1.7%,葡萄牙假丝酵母菌4株占0.95%,酿酒假丝酵母菌3株占0.71%,其他假丝酵母菌7株占1.7%。420株深部真菌对5-氟胞嘧啶和两性霉素B敏感性分别是93.1%和95.2%,氟康唑敏感率51.4%,中介率6.7%,耐药率41.9%;伊曲康唑敏感率35%,中介率15%,耐药率50%;白色假丝酵母菌对5-氟胞嘧啶和两性霉素B耐药率都是1.85%,氟康唑耐药率46.3%,中介率1.85%,伊曲康唑耐药率51.9%,中介率3.7%;非白色假丝酵母菌对5-氟孢嘧啶和两性霉素耐药率分别是26.7%和33.3%,氟康唑耐药率30.7%,中介率12%,依曲康唑耐药率46%,中介率20.6%。结论铜陵地区深部真菌种类与其他地区相同,而耐药性有差别,定期对本地区深部真菌耐药性进行监测,对合理使用抗真菌药物,延缓抗真菌药物使用周期,减少难治性真菌的产生和流行有重要临床指导价值。
Objective To understand the clinical classification of deep fungi and their drug resistance in Tongling, Anhui Province. Methods From January to December 2006, the deep fungi were isolated clinically in Tongling area and identified by BioMerieux kit API20CAUX. ATB-FUNGus21NT was tested for susceptibility. Results 270 strains of Candida albicans accounted for 64.3%, Candida tropicalis 75 strains accounted for 17.8%, Candida kudai strains accounted for 4.76%, Candida parapsilosis 18 strains accounted for 4.3% 16 yeasts accounted for 3.8%, Candida albicans 7 strains accounted for 1.7%, Portugal Candida 4 accounted for 0.95%, 3 strains of Candida albicans accounted for 0.71%, other Candida 7 strains Accounting for 1.7%. The susceptibility rate of fluconazole was 51.4%, the rate of mediation was 6.7% and the rate of resistance was 41.9%. The sensitivity of itraconazole 35%, the rate of 15%, the rate of drug resistance 50%; Candida albicans resistance to 5-fluorocytosine and amphotericin B were 1.85%, fluconazole resistance rate of 46.3%, mediated rate of 1.85 %, The drug resistance rate of itraconazole was 51.9% and the rate of mediation was 3.7%. The drug resistance rates of non-Candida albicans to 5-fluorosporine and amphotericin were 26.7% and 33.3% 30.7%, mediating rate 12%, itraconazole resistance rate 46%, mediating rate 20.6%. Conclusion The species of fungi in the deep area of Tongling are the same as those in other areas, but the drug resistance is different. The fungal resistance in the deep area of the area is monitored periodically. The rational use of antifungal agents, the delay of the antifungal drug use period and the reduction of refractory fungi And epidemiology has important clinical value.
