目的 探讨止血带对血凝纤溶及血液流变学的影响。方法 成年狗 30只 ,分为橡皮管止血带组、环绕充气止血带组、局部充气止血带组 3组 ,每组 10只。止血带连续应用 6小时后解除 ,分别测定用前、用后 3、6小时和解除止血带 5、10、30分钟及 1、2、3小时全身和实验侧肢体静脉血的凝血酶原时间、凝血酶时间、纤维蛋白原、鱼精蛋白、血液粘度和血浆渗透压。结果 在狗后肢连续应用止血带 6小时 ,应用或解除止血带时 ,实验侧肢体静脉血凝血酶原时间、凝血酶时间均比应用前明显延长 (P <0 .0 5 )。纤维蛋白原稍下降 ,但仍在正常范围之内。鱼精蛋白实验为阴性。橡皮管止血带对血凝系统的影响最大 ,环绕充气止血带次之 ,局部充气止血带组影响较轻 ,但各组间无显著差别 (P >0 .0 5 )。结论 应用止血带后 ,实验侧肢体静脉血浆渗透压显著升高 (P <0 .0 5 ) ,血液粘度上升 ,解除止血带后趋於恢复。这些血液流变学指标的改变可能均和血管通透性增高、血管内液外流、组织水肿有关。应用止血带后血凝时间延长、纤溶系统亢进 ,同时出现渗透压升高和血液粘度上升
Objective To investigate the effect of tourniquet on hemagglutination and hemorheology. Methods Thirty adult dogs were divided into rubber tube tourniquet group, inflatable tourniquet group, local inflatable tourniquet group 3 groups, 10 rats in each group. The tourniquet was released after 6 hours of continuous application. The prothrombin time of the systemic and experimental limbs was measured at 3, 6 hours after operation and at 5, 10, 30 minutes and 1, 2, 3 hours after operation. Thrombin time, fibrinogen, protamine, blood viscosity and plasma osmotic pressure. Results When the tourniquet was continuously applied to the hind limb of the dog for 6 hours, the prothrombin time and thrombin time in the extremity of the experimental group were significantly longer than those before application (P <0.05) when the tourniquet was applied or released. Fibrinogen slightly decreased, but still within the normal range. Protamine experiments were negative. The rubber band tourniquet had the greatest effect on the coagulation system, followed by the inflatable tourniquet, and the local inflating tourniquet group had less effect, but there was no significant difference among the groups (P> 0.05). Conclusion After the tourniquet was applied, the osmotic pressure of venous blood of experimental limbs increased significantly (P <0.05), the blood viscosity increased, and the tourniquet tended to recover after the tourniquet was removed. These changes in blood rheology indicators may be associated with increased vascular permeability, intravascular fluid outflow, tissue edema.
