目的探讨EB病毒(EBV)感染与淋巴瘤基因组不平衡改变、临床表现、病理类型、疗效和预后的关系。方法免疫组化法检测EBV潜伏膜蛋白(LMP-1),原位杂交方法检测EBV编码的小RNA(EBERs)表达情况,比较基因组杂交(CGH)进行基因组水平不平衡改变的研究。结果EBV感染的霍奇金淋巴瘤(HL)和胃黏膜相关淋巴瘤(MALT)型基因组无不平衡改变。外周T淋巴细胞非霍奇金淋巴瘤(NHL)表现为基因异常的数目多,核型复杂,预后不良。大B细胞NHL、小B细胞NHL表现为基因组水平缺失与增益的数目少、化疗效果好。结论EBV感染后的NHL宿主细胞基因组水平改变,扩增与/或缺失数目多,临床表现呈进展性,化疗效果差,预后不良。
Objective To investigate the relationship between Epstein-Barr virus (EBV) infection and lymphoma genomic imbalance, clinical manifestation, pathological type, curative effect and prognosis. Methods Immunohistochemical method was used to detect the expression of EBV latent membrane protein (LMP-1), the expression of EBV-encoded small RNAs (EBERs) by in situ hybridization and the comparison of genome-wide hybridization (CGH) Results EBV-infected Hodgkin’s lymphoma (HL) and gastric mucosa-associated lymphoma (MALT) genotype had no unbalanced changes. Peripheral T lymphocyte non-Hodgkin’s lymphoma (NHL) showed the number of gene abnormalities, karyotype complex, the prognosis is poor. Large B cell NHL, small B cell NHL showed a small number of genomic levels of loss and gain, the effect of chemotherapy is good. Conclusion The genomic level of NHL host cells after EBV infection is changed, the number of amplification and / or deletion is large, the clinical manifestations are progressive, the effect of chemotherapy is poor, and the prognosis is poor.
