The incidence of blindness associated with midfacial fractures ranged from 0.3% to 3.5% due to damage to the eyeball, optic nerve or visual pathway. Most cases are caused by damage to the optic nerve within the optic canal and such damage can be direct or indirect. Walsh and Hoyte to indirect optic nerve injury is defined as: traumatic vision loss, no external or primary ophthalmological evidence of eye or optic nerve damage. Indirect optic nerve injury (ischemic neuropathy) is caused by oppression of the optic nerve after the bleeding or peripheral edema caused by oppression, or secondary to vasospasm or thrombosis. The authors retrospectively analyzed patients with midfacial fractures from 1995 to 1997, except for simple nasal bone fractures. All cases of vision loss were evaluated by maxillofacial surgery, pathology and neurosurgeons.
