目的:探讨P钙黏蛋白(P-cadherin)与热休克蛋白27(HSP27)在胃癌组织中的表达及其与临床病理因素之间的关系,进而探讨其作为预后指标的意义。方法:建立组织芯片,通过免疫组化SP法检测75例无术前放化疗史的胃癌手术标本和75例正常胃组织中P-cadherin、HSP27的表达情况,并对随访患者进行生存分析。结果 :在胃癌组织及正常胃组织中,Pcadherin、HSP27的表达率分别为40.0%、82.7%(P<0.001)和64.0%、30.7%(P<0.001)。P-cadherin低表达和HSP27高表达与临床分期、分化程度和淋巴结转移相关(P<0.05),HSP27高表达还与肿瘤大小相关(P<0.05)。Pearson相关分析显示,P-cadherin与HSP27在胃癌中的表达呈负相关(P<0.05)。单因素Kaplan-Meier回归分析显示P-cadherin和HSP27与预后相关(P<0.05)。Cox单因素回归分析显示P-cadherin、HSP27、临床分期、肿瘤大小、有无淋巴结转移与患者预后相关(P<0.05);Cox多因素回归模型分析显示HSP27、临床分期与患者预后相关(P<0.05)。结论:在胃癌中,P-cadherin低表达、HSP27高表达与胃癌临床病理特征和生物学行为密切相关。P-cadherin与HSP27的表达在胃癌组织中呈负相关。P-cadherin和HSP27可作为判断患者预后的候选指标。
Objective: To investigate the expression of P-cadherin and heat shock protein 27 (HSP27) in gastric carcinoma and its relationship with clinicopathological factors, and to explore its significance as a prognostic indicator. Methods: Tissue microarray was used to detect the expression of P-cadherin and HSP27 in 75 cases of gastric cancer without surgery and 75 cases of normal gastric tissue by immunohistochemical SP method. Survival analysis was performed in the follow-up patients. Results: The expressions of Pcadherin and HSP27 were 40.0%, 82.7% (P <0.001) and 64.0%, 30.7% (P <0.001) respectively in gastric cancer and normal gastric tissue. Low expression of P-cadherin and high expression of HSP27 correlated with clinical stage, differentiation degree and lymph node metastasis (P <0.05), and high expression of HSP27 correlated with tumor size (P <0.05). Pearson correlation analysis showed that P-cadherin and HSP27 expression in gastric cancer was negatively correlated (P <0.05). Univariate Kaplan-Meier regression analysis showed that P-cadherin and HSP27 were correlated with prognosis (P <0.05). Cox univariate regression analysis showed that P-cadherin, HSP27, clinical stage, tumor size and lymph node metastasis were correlated with prognosis (P <0.05). Cox regression analysis showed that the clinical stage of HSP27 correlated with the prognosis of patients (P < 0.05). Conclusion: The low expression of P-cadherin and the high expression of HSP27 in gastric cancer are closely related to the clinicopathological features and biological behavior of gastric cancer. P-cadherin and HSP27 expression in gastric cancer was negatively correlated. P-cadherin and HSP27 can be used as prognostic indicators in patients with prognosis.
