目的检测B7-H1在胰腺癌中的表达情况,初步探讨其与胰腺癌临床病理因素及预后的关系。方法应用免疫组化法和Western-blotting检测46例胰腺癌组织和5例正常胰腺组织中B7-H1的表达情况,统计学分析B7-H1和胰腺癌临床病理学特征和预后之间的关系。结果免疫组化结果发现46例胰腺癌标本中,B7-H1阳性率为56.5%(26/46),而正常胰腺组织中,未发现B7-H1阳性显色。Western-blotting结果显示在胰腺癌组织中B7-H1蛋白呈高表达(0.512±0.125);正常胰腺组织仅微弱表达(0.122±0.027),P<0.05。结合分析患者临床资料发现,B7-H1的表达与淋巴结转移、TNM分期和病理分化程度相关(P<0.05),与患者年龄、性别、肿瘤部位和大小无关。Spearman相关性分析提示肿瘤组织中B7-H1表达与血清CA19-9表达有正相关性(r=0.63)。生存曲线结果显示B7-H1阴性组胰腺癌患者较阳性组生存期明显延长(P<0.05)。Cox回归多因素分析显示B7-H1的表达是影响胰腺癌患者预后的独立因素。结论人胰腺癌组织中存在B7-Hl高表达并与胰腺癌患者的预后相关,可能是胰腺癌病情评估和预后监测的独立指标。
Objective To detect the expression of B7-H1 in pancreatic cancer and to explore its relationship with clinicopathological factors and prognosis of pancreatic cancer. Methods The expression of B7-H1 in 46 pancreatic cancer tissues and 5 normal pancreatic tissues was detected by immunohistochemistry and Western-blotting. The relationship between the clinicopathological features and prognosis of B7-H1 and pancreatic carcinoma was statistically analyzed. Results The positive rate of B7-H1 in 46 cases of pancreatic cancer was 56.5% (26/46). However, in normal pancreas, no positive staining of B7-H1 was found. Western-blotting results showed that B7-H1 protein was highly expressed in pancreatic cancer tissues (0.512 ± 0.125); normal pancreatic tissue was only weakly expressed (0.122 ± 0.027), P <0.05. According to the clinical data of patients, B7-H1 expression was correlated with lymph node metastasis, TNM stage and pathological differentiation (P <0.05), but not with age, sex, tumor location and size. Spearman correlation analysis showed that there was a positive correlation between B7-H1 expression and serum CA19-9 expression (r = 0.63). Survival curves showed that patients with B7-H1 negative pancreatic cancer had significantly longer survival (P <0.05) than patients with positive pancreatic cancer. Cox regression multivariate analysis showed that B7-H1 expression was an independent factor affecting the prognosis of patients with pancreatic cancer. Conclusion The high expression of B7-H1 in human pancreatic cancer is associated with the prognosis of patients with pancreatic cancer, which may be an independent indicator of disease progression and prognosis in pancreatic cancer.
