为探讨狼疮性肾炎(LN)患者外周血淋巴细胞亚群、活化状态和免疫球蛋白的变化及其相互关系,应用流式细胞仪和酶联检测仪,对21例LN进行了研究。结果发现:①LN患者CD3~++CD4~+、CD45RA~+、CD45RO~+、CD4~++CD45RA~+、CD4~++CD45RO~+细胞比例明显减少(与对照组比较,P<0.05、P<0.01或P<0.001)。②HLA-DR~+、CD19~++HLA-DR~+、CD19~++CD43~+、CD+19~++CD5~+细胞比例明显增高(与对照组比较,P<0.05、P<0.01或P<0.001)。③LN患者IgE水平明显升高(与对照组相比,肾病综合征(NS)组P<0.001,非NS组P<0.05,NS组与非NS组比较,P<0.05);IgM、IgA水平也升高(P<0.05),NS组与非NS组比较,P>0.05;IgG NS组明显降低,而非NS组明显升高,P均<0.001,NS组与非NS组比较P<0.001。结论:LN患者外周血T淋巴细胞亚群及免疫球蛋白的变化与系统性红斑狼疮患者并不完全相同,甚至出现截然相反的结果;LN患者免疫功能的紊乱与淋巴细胞活化及其亚群之间的协调与失调密切相关;LN患者存在B细胞过度活化,由此而产生的自身多克隆抗体是引发本病的根本原因。
To investigate the changes of peripheral blood lymphocyte subsets, activation status and immunoglobulins in patients with lupus nephritis (LN) and their correlations, 21 cases of LN were studied by flow cytometry and enzyme-linked detector. The results showed that: (1) The proportion of CD3 + CD4 +, CD45RA +, CD45RO +, CD4 ~ + CD45RA ~ + and CD4 ~ + CD45RO ~ + cells in LN patients decreased significantly P <0.01 or P <0.001). ② The proportion of HLA-DR +, CD19 + + HLA-DR +, CD19 + + CD43 +, CD + 19 ~ + + CD5 + cells was significantly higher than that of control group (P <0.05, P <0.01 Or P <0.001). (3) The level of IgE in LN patients was significantly higher than that in control group (P <0.001 in NS group, P <0.05 in non-NS group, P <0.05 in NS group and non-NS group) (P <0.05). Compared with non-NS group, P> 0.05; NS-NS group was significantly lower than NS-NS group, P <0.001, P <0.001. CONCLUSION: The changes of T lymphocyte subsets and immunoglobulins in peripheral blood of patients with LN are not exactly the same as that of patients with systemic lupus erythematosus, even with the opposite results. The immune dysfunction and lymphocyte activation in LN patients and their subsets There is a close relationship between the coordination and imbalance; LN patients with excessive activation of B cells, the resulting polyclonal antibodies is the root cause of this disease.
