目的 分析肿瘤异常蛋白(TAP)在大肠息肉(CRP)、大肠癌(CRC)患者中的表达水平,初步探讨TAP对CRC及其癌前病变的诊断价值和病情评估作用.方法 采用回顾性分析的方法,收集苏州市立医院东区消化内科、胃肠肿瘤外科2017年10月—2018年8月收治的CRP患者63 例、CRC患者75 例为研究对象,同期查体的健康成人42 名为对照,分析TAP的检测结果,探讨CRP、CRC的发病年龄、性别、病灶大小、生长部位、病理分型等临床特点与TAP的相关性.结果近1年来,健康成人、CRP患者、CRC患者的TAP阳性率分别为2.38%、14.29%、73.33%.TAP在CRC检测中,灵敏度亦高于肿瘤标志物CEA.CRP的发病年龄、性别、息肉个数、大小、病理分型对TAP无影响(P>0.05),但直肠、乙降结肠息肉患者的TAP阳性率显著高于横结肠、升结肠息肉患者(P<0.05).CRC患者的年龄、性别、肿瘤大小、分化程度与TAP无关(P>0.05),但Ⅳ期TAP阳性率显著高于Ⅰ-Ⅱ期、Ⅲ期(P<0.05),而不同分期患者的TAP凝集面积差异无统计学意义(P>0.05),此外直肠癌患者的TAP阳性率也明显高于结肠癌患者(P<0.05).结论 TAP在正常人群、CRP患者、CRC患者中阳性率呈递增趋势,直肠息肉、直肠癌患者的TAP阳性率更高.同时,TAP检测CRC时较CEA具有更高的灵敏度.提示TAP联合CEA检测有助于早期干预CRC癌前病变,并作为CRC早期筛查的一种重要手段.,Objective To analyze the expression of serum tumor abnormal protein (TAP) in colorectal polyp (CRP) and colorectal cancer (CRC) patients, and then to preliminarily explore the value of TAP to diagnosis and evaluation of CRC and precancerous lesion. Methods From October 2017 to August 2018 in the East District of Suzhou Municipal Hospital, 63 cases of CRP, 75 cases of CRC and 42 normals were collected for the study with retrospective analysis method. After comparison of TAP results among these three groups, the correlation between TAP and clinical features of age, sex, lesion size, site, histopathological types, et al, were investigated in CRP and CRC patients. Results In recent 1 year, the positive rates of TAP in normals, CRP and CRC cases were 2.38%, 14.29% and 73.33%. TAP was also much more sensitive than CEA to CRC. In CRP group, TAP results were independent of age, sex, polyp number, size and histopathological types (P>0.05). The positive rate of TAP in rectal, sigmoid colon and/or descending colon polyp patients was much higher than transverse colon and/or ascending colon polyp patients (P<0.05). On the other hand, there were no obvious correlations between TAP and age, sex, tumor size, differentiation in CRC group (P>0.05). The positive rate of TAP in stageⅣ was significantly higher than in stage Ⅰ-Ⅱ and stage Ⅲ, which was also higher in rectal cancer compared with colon cancer (P<0.05). Conclusion The positive rates of TAP increase in order of normals, CRP patients to CRC patients. It’s much higher of TAP positive rate in rectal polyp and rectal cancer sufferers. Because of high sensitivity, the combining detection of TAP and CEA may contribute to early intervention in precancerous lesion of CRC, and further to be an important method for CRC screening.
