目的 :观察白内障患者超声乳化手术前后泪膜的变化和应用人工泪液后对泪膜的影响。方法 :6 0例患者随机分为两组 ,A组超声乳化术后单用典必殊眼液滴眼 ,B组联合应用典必殊和泪然眼液滴眼 ,分别于手术前、手术后 1天、1周、1个月、3个月对 6 0例患者行棉线检查 ,角膜荧光素染色、泪膜破裂时间 (BUT)和SchirmerⅠ检查 ,对结果做统计学分析。结果 :(1)手术前A组和B组在棉线试验、角膜染色、BUT、SchirmerⅠ四个检查值经统计学分析无明显差异 ;(2 )两组术后 1天B组角膜染色分值小于A组 ,并且BUT较A组更长 ,统计学上有显著性差异 ,棉线试验和SchirmerⅠ试验检查B组虽有延长 ,但在统计学上没有显著性差异 ;(3)手术后 1周B组泪膜破裂时间比A组延长 ,差异有显著性 ,角膜染色、棉线试验和SchirmerⅠ试验检查两组在统计学上没有显著性差异 ;(4 )手术后 1个月B组在角膜染色评分小于A组 ,统计学上有显著差异 ,棉线试验、BUT和SchirmerⅠ两组无显著性差异 ;(5 )手术后 3个月B组角膜染色分值明显小于A组 ,泪膜破裂时间明显延长 ,统计学上有显著差异 ,棉线试验和SchirmerⅠ试验检查两组在统计学上没有显著性差异。结论 :白内障超声乳化术后早期泪膜稳定性明显下降 ,手术后泪膜脂质层和粘液层受到的损害影响了泪膜的稳定
Objective: To observe the changes of tear film before and after phacoemulsification in cataract patients and the effect of artificial tear on tear film. Methods: Totally 60 patients were randomly divided into two groups. Group A was treated with phacoemulsification alone, while group B was treated with Dictxe and Tear Eye Drops before operation and after operation Sixty patients underwent cotton thread examination, corneal fluorescein staining, tear film break-up time (BUT) and Schirmer I test at 1 day, 1 week, 1 month and 3 months. The results were statistically analyzed. Results: (1) There was no significant difference in the four test values of cotton thread test, corneal staining, BUT and Schirmer Ⅰ before operation between groups A and B; (2) Corneal staining score of group B was less than A group, and the BUT longer than the A group, statistically significant differences, cotton thread test and Schirmer Ⅰ test to check the B group has been extended, but no statistically significant difference; (3) 1 week after surgery in group B The tear film rupture time was longer than that in group A, the difference was significant. Corneal staining, cotton thread test and Schirmer Ⅰ test showed no statistically significant difference between the two groups. (4) The corneal staining score of group B was less than A There was no significant difference between the two groups in cotton thread test, BUT and Schirmer Ⅰ. (5) The corneal staining score of group B at 3 months after operation was significantly lower than that of group A, and tear film rupture time was significantly prolonged. Statistics There was a significant difference between the cotton thread test and Schirmer I test in the two groups showed no statistically significant differences. CONCLUSIONS: Early tear film stability is significantly reduced after phacoemulsification, and tear film stability is affected by damage to the tear film lipid layer and mucus layer after surgery
