采用PCR和RFLP方法分析乙型重型肝炎前C区1896位变异情况,同时测定血清中一些细胞因子水平。结果为(1)HBV前C区变异组的IFNγ,IL6水平明显高于非变异组。(2)TNFα,IL8在变异组和非变异组间无明显差异。(3)变异组中8/10例死亡,非变异组1/9例死亡。结果提示;HBV前C区1896位变异使重型肝炎患者T淋巴细胞的免疫调节作用发生异常,出现T淋巴细胞产生细胞因子能力异常增高,与重型肝炎病情加重恶化有关。
PCR and RFLP method were used to analyze the variation of 1896 positions in pre-C region of severe hepatitis B, and some serum levels of cytokines were also measured. The results were (1) pre-C mutation in HBV group IFN γ, IL 6 levels were significantly higher than non-mutation group. (2) TNF α, IL 8 in the mutation group and non-mutation group no significant difference. (3) 8/10 deaths in the mutation group and 1/9 deaths in the non-mutation group. The results suggest that 1896 mutations in pre-HBV C region lead to an abnormal immune regulation of T lymphocytes in patients with severe hepatitis. The abnormal ability of T lymphocytes to produce cytokines appears abnormally, which is related to the worsening of severe hepatitis.
