目的评估一种新的MRI水脂分离技术——屏气容积内插法梯度回波T2*衰减校正水脂分离技术(简称3D GRE MRI)在诊断脂肪肝中的作用。方法 40例受检者在1.5TMR扫描仪进行肝脏常规MRI、单体素1H-MRS和3D GRE MRI检查,2位MRI医师根据图像共同分析肝脏MR图像表现和MRS谱线特征,判断有无脂肪肝。以MRS诊断结果为参考标准,评价3D GRE MRI在诊断脂肪肝中的作用。结果①40例受检者中MR和MRS诊断脂肪肝者21例,无脂肪肝者19例。脂肪肝程度越重,同/反相位图上肝脏实质信号差别越明显,在脂肪分数含量图上信号也越高,测量的HFF3D GRE MRI值也越高。对照组21例,HFF3D GRE MRI在2.43%~12.60%,中位数4.55%;脂肪肝组19例,HFF3D GRE MRI在12.35%~38.0%,中位数18.13%。②HFF3D RGRE MRI和1H-MRS所测量的脂肪分数HFFMRS呈线性正相关,Spearson秩相关系数为0.963(P<0.01)。结论 3D GRE MRI T2*衰减校正水脂分离技术为脂肪肝的诊断提供了一种新的方法,一次屏气完成全肝检查,有潜力在高危人群中筛查脂肪肝,对治疗效果进行评价等方面发挥重要作用。
Objective To assess the role of a new MRI water-fat separation technique, breath-hold volume interpolation gradient echo T2 * attenuation-corrected water-lipid separation (3D-GRE MRI) in the diagnosis of fatty liver. Methods Forty subjects underwent routine liver MRI, 1H-MRS and 3D GRE MRI examinations on a 1.5T MRI scanner. Two MRI physicians analyzed the liver MR images and the characteristics of MRS according to the images to determine the presence or absence of fat liver. MRS diagnostic results as a reference standard to evaluate the 3D GRE MRI in the diagnosis of fatty liver. Results ① Forty patients were diagnosed with MR and MRS in 21 cases of fatty liver and 19 cases of non-fatty liver. The more severe the fatty liver, the more obvious the difference of the real liver signal in the same / opposite phase diagram, and the higher the signal in the fat fraction map, the higher the measured HFF3D GRE MRI value. The control group consisted of 21 patients with HFF3D GRE MRI ranging from 2.43% to 12.60% with a median of 4.55%. In the fatty liver group, 19 patients had HFF3D GRE MRI ranging from 12.35% to 38.0% with a median of 18.13%. HFFMRS was positively correlated with HFFMRS measured by HFF3D RGRE MRI and 1H-MRS, and Spearson rank correlation coefficient was 0.963 (P <0.01). Conclusion The 3D GRE MRI T2 * attenuation-corrected water-fat separation technique provides a new method for the diagnosis of fatty liver. The first time, the whole liver examination is completed, the potential is to screen fatty liver in high-risk groups and evaluate the therapeutic effect Play an important role.
