目的探讨标本溶血对生化检验结果的影响。方法随机选取我院2014年6月-2016年6月收治的80名正常人的血液标本作为研究对象,分别检测溶血前和溶血后血清中的总胆红素(TBIL)、胆红素(DBIL)、丙氨酸氨基转氨酶(ALT)、门冬氨酸氨基转移酶(AST)、清蛋白(ALB)、尿素氮(BUN)、肌酐(Cr)、肌酸激酶(CK)等生化指标,对比分析各指标在溶血前后的变化。结果溶血对总胆红素(TBIL)、胆红素(DBIL)、丙氨酸氨基转氨酶(ALT)、门冬氨酸氨基转移酶(AST)、清蛋白(ALB)、肌酸激酶(CK)影响显著,差异具有统计学意义(P<0.05);对尿素氮(BUN)、肌酐(Cr)影响不明显,差异无统计学意义(P>0.05)。结论生化指标在溶血后升高的可能影响因素是溶血时血液标本中酶活性增强,生化指标在溶血后下降的可能原因是溶血引起的稀释作用或相关检测试剂受溶血的影响不大。在临床工作中应该警惕标本出现溶血现象,提高检验的准确性和诊断的可靠性。
Objective To investigate the effect of hemolysis on the biochemical test results. Methods A total of 80 normal subjects from June 2014 to June 2016 in our hospital were selected as the study subjects. The levels of total bilirubin (TBIL), bilirubin (DBIL) ), ALT, ALB, ALB, BUN, Cr, creatine kinase (CK) Analyze the changes of each index before and after hemolysis. Results Hemolysis had no significant effect on TBIL, DBIL, ALT, ALB, CK, The difference was statistically significant (P <0.05). The effects on BUN and Cr were not significant (P> 0.05). Conclusions The possible influencing factors of the increase of biochemical indexes after hemolysis are the increase of enzyme activity in hemolytic blood samples and the decrease of biochemical indexes after hemolysis. The possible reasons are dilution effect caused by hemolysis or little influence of hemolysis on related detection reagents. In clinical work should be vigilant specimen hemolysis, improve the accuracy of the test and the reliability of diagnosis.
