目的探讨新疆某三级甲等综合性医院包虫病患者住院费用的影响因素,为医疗费用控制提供参考依据。方法选择某医院2010年-2014年之间因包虫病住院的患者为研究对象,收集患者住院费用及相关资料,使用多重线性及通径分析从多因素的角度分析其医疗费用的影响因素。结果经多重线性回归分析,包虫病患者住院费用的影响因素回归方程为:Y=3.272+0.018X_9+0.001X_2+0.077X_4+0.549X_(10)-0.089X_5+0.497X_6+0.003X_(11)(R~2=0.75,F=326.29,P=0.000);对住院费用的通径分析显示,该模型对包虫病住院费用的总解释度为75%;通过对各因素作用系数比较得知,影响包虫病住院费用的因素包括否手术、住院天数、药费比、转科、其他诊断条目、年龄等(P均<0.05)。结论本次研究分析显示手术及住院天数是包虫病患者住院费用的主要影响因素。该研究可为分级阶梯式的包虫病补助政策和该病种DRGs分型提供参考依据,以进一步推动新疆地区的包虫病防治工作。
Objective To explore the influencing factors of hospitalization costs of hydatid disease patients in a three-level general hospital in Xinjiang, and to provide a reference for the control of medical expenses. Methods The patients hospitalized for hydatid disease from 2010 to 2014 in a hospital were selected as the study objects. The hospitalization expenses and related information were collected. Multiple linearity and path analysis were used to analyze the influencing factors of medical expenses from multiple factors. Results Multiple linear regression analysis showed that the regression equation of the influencing factors of hospitalization costs of patients with hydatid disease was Y = 3.272 + 0.018X_9 + 0.001X_2 + 0.077X_4 + 0.549X_ (10) -0.089X_5 + 0.497X_6 + 0.003X (11) (R ~ 2 = 0.75, F = 326.29, P = 0.000). The path analysis of hospitalization cost showed that the model explained 75% of the total cost of hospitalization for hydatid disease. By comparing the effect coefficient of each factor The factors influencing the hospitalization cost of echinococcosis include the number of days of hospitalization, the number of days of hospitalization, the cost of medicine, transfer of subjects, other diagnostic items and age (all P <0.05). Conclusion This study shows that the number of days of surgery and hospitalization is the main influencing factor for the hospitalization expenses of patients with hydatid disease. This study can provide a reference for grading ladder-type hydatid disease subsidy policy and DRGs typing of the disease, so as to further promote the prevention and control of hydatid disease in Xinjiang.
