目的探讨经颅多普勒超声(TCD)对脑梗死的诊断价值及临床意义。方法选择经神经系统症状与头部CT或MR I检查确诊的58例脑梗死患者,应用TCD检测血流变化情况,并与正常对照组32例进行比较。结果脑梗死患者TCD检查病变区血流表现为减慢、加快或消失,以血流减慢多见,占39.7%;脑梗死组血流异常率(67.2%)明显高于对照组(18.8%)(P<0.05);TCD检查定位结果与影像学检查符合率为86.0%。结论 TCD具有自身优势及局限性,对脑梗死早期诊断、病情评估及治疗和预后有重要临床价值,可作为脑梗死的重要辅助诊断手段。
Objective To investigate the diagnostic value and clinical significance of transcranial Doppler ultrasound (TCD) in cerebral infarction. Methods Fifty-eight patients with cerebral infarction who were diagnosed by neurological symptoms and head CT or MR I were selected. The changes of blood flow were detected by TCD and compared with those in the normal control group (n = 32). Results The blood flow of TCD in the patients with cerebral infarction was slowed down, accelerated or disappeared, and the blood flow was more common, accounting for 39.7%. The rate of abnormal blood flow in the cerebral infarction group was significantly higher than that in the control group (18.8% ) (P <0.05). The coincidence rate of TCD examination and imaging examination was 86.0%. Conclusion TCD has its own advantages and limitations. It has important clinical value in early diagnosis, assessment of disease, treatment and prognosis of cerebral infarction, and can be used as an important auxiliary diagnosis method for cerebral infarction.
