目的探讨黄连素联合阿托伐他汀钙对脑梗死患者微栓子的影响。方法按随机数字表法将100例脑梗死患者分为观察组和对照组各50例,两组均进行常规治疗,对照组给予阿托伐他汀钙20 mg口服,1次/d;观察组在对照组的基础上加用黄连素0.4 g口服,3次/d,0.4 g/次;两组均治疗3个月。治疗前、治疗后3个月进行NIHSS评分,采用经颅多普勒超声检测微栓子数量,采用高频(LOGIQ 9)彩色多普勒超声检测颈动脉不稳定斑块数目。采用Olympus-AU640全自动生化分析仪检测治疗前后的AST、ALT、肌酐(s Cr)。结果观察组和对照组治疗3个月后微栓子数量、NIHSS评分及不稳定斑块数均较治疗前明显下降,差异有统计学意义(P均<0.01)。治疗后3个月,观察组微栓子数量、NIHSS评分及不稳定斑块数均明显低于对照组,差异有统计学意义(P均<0.01)。两组治疗前后AST、ALT、s Cr比较,P均>0.05。结论黄连素联合阿托伐他汀钙能够减少微栓子数量,抑制不稳定斑块的形成,且无不良反应。
Objective To investigate the effect of berberine combined with atorvastatin on microemboli in patients with cerebral infarction. Methods 100 cases of cerebral infarction were divided into observation group and control group according to random number table method, 50 cases in each group. Both groups were treated routinely. The control group was orally administered with 20 mg of atorvastatin calcium once a day. On the basis of the control group, berberine 0.4 g was orally administered 3 times / day and 0.4 g / time; both groups were treated for 3 months. NIHSS scores were measured before treatment and 3 months after treatment. The number of micro-emboli was detected by transcranial Doppler and the number of unstable carotid plaques was detected by high frequency (LOGIQ 9) color Doppler ultrasound. The AST, ALT and creatinine (Cr) before and after treatment were detected by Olympus-AU640 automatic biochemical analyzer. Results The number of microemboli, the number of NIHSS and the number of unstable plaques in the observation group and the control group after 3 months of treatment were significantly lower than those before treatment (P all <0.01). At 3 months after treatment, the number of micro-emboli, the number of NIHSS and the number of unstable plaques in observation group were significantly lower than those in control group (all P <0.01). AST, ALT, s Cr before and after treatment in both groups, P> 0.05. Conclusion Berberine combined with atorvastatin calcium can reduce the number of micro-emboli, inhibit the formation of unstable plaques, and no adverse reactions.
