目的:探讨肺通气功能中的各项指标应用于慢性阻塞性肺疾病(COPD)的诊断效果。方法:选取2009年1月~2010年1月来我院就诊的被确诊为COPD的患者100例,以回顾性分析的方式对其肺通气功能中的各项指标进行分析。结果:COPD不同程度患者的各项指标同对照组相比,有一定程度的降低(P<0.001);COPD各组间一秒量(FEV1),全部具有统计学差异(P<0.001);COPD轻度患者25%呼气流速(V25)、50%呼气流速(V50)、最大呼气中期流速(MMEF)同对照组比较有显著降低(P<0.001);除轻度外,其他三种程度COPD患者的肺活量(VC)同其他组比较,下降非常明显(P<0.001);每组的最大分钟通气量(MVV)、一秒量以及呼气峰流速度(PEF)都具有较强的正相关性。结论:肺通气功能已经成为COPD诊断中的重要标准,其中,V25、V50和MMEF为COPD诊断的敏感性指标,而MVV、PEF和FEV1则是判断COPD气流阻塞程度的相关指标。
Objective: To investigate the diagnostic value of various indexes of pulmonary ventilation in chronic obstructive pulmonary disease (COPD). Methods: A total of 100 patients diagnosed as COPD from January 2009 to January 2010 were retrospectively analyzed. The indexes of pulmonary ventilation were analyzed. Results: The indexes of patients with different degrees of COPD were lower than those of the control group (P <0.001), the one second volume (FEV1) of COPD between groups was statistically significant (P <0.001) Mild patients 25% expiratory flow (V25), 50% expiratory flow (V50), the maximum expiratory flow rate (MMEF) compared with the control group were significantly lower (P <0.001); except mild, the other three The level of vital capacity (VC) in patients with COPD was significantly lower than that in other groups (P <0.001). Among them, V25, V50 and MMEF are the sensitive indexes for the diagnosis of COPD, while MVV, PEF and FEV1 are the related indexes to judge the obstruction of COPD.
