目的观察兔单侧输尿管部分梗阻(partial unilateral ureteral obstruction,PUUO)后不同时期的血清半胱氨酸蛋白酶抑制剂C(Cystatin C,Cys C)、β2-微球蛋白(β2-microglobulin,β2-MG)、尿酸(Uric Acid,UA)的变化,探讨联合测定血清Cys C、β2-MG、UA在肾积水肾功能损害的诊断、治疗和恢复中的应用价值。方法建立PUUO模型,检测兔血清Cys C、β2-MG、UA。以核素肾动态显像测得的肾小球滤过滤(GFR)为标准,比较Cys C、β2-MG、UA与GFR的相关性。结果 PUUO模型的兔血清Cys C、β2-MG、UA在输尿管部分梗阻后1周与对照组相比差异无统计学意义(P>0.05),梗阻后2周、4周、8周不同时间点均显著高于对照组(P<0.01);血清Cys C、β2-MG、UA与GFR存在着显著相关关系。结论血清Cys C、β2-MG、UA是反映GFR较理想的血清标志物,对肾积水肾功能损害程度的判断具有极其重要的意义,动态观察可用于指导临床上肾积水肾功能损害的诊断、治疗和恢复情况。
Objective To observe the effects of different concentrations of Cystatin C (Cys C) and β2-microglobulin (β2-MG) in rabbits after unilateral ureteral obstruction (PUUO) ) And uric acid (UA) in patients with hydronephrosis, and to explore the value of combined determination of serum Cys C, β2-MG and UA in the diagnosis, treatment and recovery of hydronephrosis. Methods PUUO model was established to detect serum Cys C, β2-MG, UA. The correlation between Cys C, β2-MG, UA and GFR was compared with GFR measured by radionuclide renal dynamic imaging. Results The serum levels of Cys C, β2-MG and UA in PUUO rabbit model at 1 week after ureteral obstruction were not significantly different from those in control group (P> 0.05), but at 2, 4 and 8 weeks after obstruction (P <0.01). There was a significant correlation between serum Cys C, β2-MG, UA and GFR. Conclusions Serum Cys C, β2-MG and UA are serum markers that reflect the ideal GFR. It is of great significance to judge the degree of renal damage caused by hydronephrosis. Dynamic observation can be used to guide clinical renal damage of hydronephrosis Diagnosis, treatment and recovery.
