目的全面了解泛耐药鲍氏不动杆菌β-内酰胺类耐药机制。方法收集2008年1-12月住院患者痰液标本中分离的泛耐药鲍氏不动杆菌20株,用聚合酶链反应(PCR)法,分析33种β-内酰胺酶基因和膜孔蛋白carO基因以及外排泵adeB基因。结果 20株泛耐药鲍氏不动杆菌共检出A类β-内酰胺酶基因TEM-1、C类β-内酰胺酶基因ADC-30、D类β-内酰胺酶基因OXA-23,三者阳性率均为100.0%;B类β-内酰胺酶基因未检出;膜孔蛋白carO基因突变率为100.0%,外排泵adeB基因阳性率为100.0%。结论泛耐药鲍氏不动杆菌株对β-内酰胺类药物耐药与产TEM-1、ADC-30、OXA-23β-内酰胺酶,膜孔蛋白carO基因突变和获得adeABC外排泵3种耐药的机制相关。
Objective To understand the mechanism of β-lactam resistance in Pan-resistant Acinetobacter baumannii. Methods Twenty isolates of Pan-drug-resistant Acinetobacter baumannii were collected from sputum samples from January to December in 2008. Polymerase chain reaction (PCR) was used to analyze 33 kinds of β-lactamase genes and membrane pore proteins carO gene and the efflux pump adeB gene. Results A group of β-lactamase gene TEM-1, class C β-lactamase gene ADC-30, class D β-lactamase gene OXA-23 were detected in 20 strains of Pan-resistant Acinetobacter baumannii, The positive rates of all the three were 100.0%. The B-type β-lactamase gene was not detected. The mutation rate of membrane porin carO gene was 100.0% and the efflux pump adeB gene was 100.0%. Conclusion Pan-drug-resistant Acinetobacter baumannii strains are resistant to β-lactam drugs and produce mutations of TEM-1, ADC-30, OXA-23β-lactamase and membrane porin carO gene and obtain adeABC efflux pump 3 The mechanisms of drug resistance are related.
