目的观察替吉奥胶囊联合三维适行放疗技术在胃癌进展期患者的临床效果及毒副作用。方法选择东阳市人民医院2010年4月—2013年4月收治的142例确诊为胃癌进展期患者,按照所在病区分为单纯放疗组69例与放化疗结合组73例,单纯放疗组患者单纯采用三维适行放疗法进行放射治疗,180 cGy/d,5次/周,照射4~8周共18~30次,总剂量4500~5500 cGy。放化疗结合组患者在此基础上给予抗肿瘤药物替吉奥胶囊口服治疗,40 mg/m2,每天早晚饭后各1次,连服5 d后休2 d,7 d为1个周期,共进行4个周期。比较2组患者治疗后的临床疗效及毒副作用。结果治疗后放化疗结合组患者CR 26例,PR 20例,临床总有效为63.01%,单纯放疗组患者CR 11例,PR 22例,临床总有效率47.83%,2组比较差异有统计学意义(P<0.05);不良反应方面,虽放化疗组患者Ⅱ~Ⅳ期血液学毒性方面毒副作用较多,但均在患者耐受内,且经过对症治疗及相应支持后症状迅速缓解,未出现严重后果。结论替吉奥胶囊联合三维适形放疗可以有效改善患者临床疗效,虽有一定毒副作用,但是均在患者临床耐受内,两者联合值得在临床广泛推广应用。
Objective To observe the clinical effects and toxicities and side effects of tigeo capsule combined with three-dimensional adaptive radiotherapy in patients with advanced gastric cancer. METHODS: A total of 142 patients diagnosed as advanced gastric cancer from April 2010 to April 2013 in Dongyang People’s Hospital were enrolled. According to their location, 69 patients were treated with radiotherapy alone and 73 patients received radiotherapy and chemotherapy. Patients in radiotherapy alone group Radiotherapy with three-dimensional adaptive radiotherapy, 180 cGy / d, 5 times / week, irradiation 4 to 8 weeks a total of 18 to 30 times, a total dose of 4500 ~ 5500 cGy. The clinical efficacy and side effects of the two groups were compared. Results In the radiotherapy and chemotherapy combined group, 26 cases were CR and 20 cases were PR. The total clinical effective rate was 63.01%. In the radiotherapy group, 11 cases were CR and 22 cases were PR, the total effective rate was 47.83%. There was significant difference between the two groups (P <0.05). In side effects, although there were more toxic and side effects in hematological toxicity of patients in radiotherapy and chemotherapy group in stage Ⅱ ~ Ⅳ, they were all within patient tolerance, and the symptom was relieved without symptomatic treatment and corresponding support Serious consequences. Conclusion TEGO capsules combined with three-dimensional conformal radiotherapy can effectively improve the clinical efficacy of patients, although some side effects, but are within the patient’s clinical tolerance, the combination of the two worthy of widespread application in the clinic.
