目的 :本研究是对腰椎各部位骨密度测量指标比较 ,从而得到一项敏感指标来随访骨质疏松病人腰椎骨密度年变化率。方法 用 HOLOGIC QDR- 2 0 0 0型 DXA骨密度仪对 5 4例骨质疏松门诊女性病人进行随访。年龄 5 8.4± 8.2岁。随访时间 3~ 36月 ,每位病人作 L2 -4 正侧位骨密度测量。同时对腰椎质控模型作正侧位扫描 ,进行质量监控。结果 ( 1)随访结果与自身对比分析 ,得到前后位面密度( AP- BMD)年变化率 1.49( 0 .46,3.86) %· a-1、侧位腰椎的面骨密度 ( L- BMD)年变化率 2 .89( 1.0 5 ,7.95 ) %· a-1、侧位腰椎的体积骨密度 ( L- VBMD)年变化率 2 .5 7( 0 .82 ,6.5 0 ) %·a-1、侧位腰椎椎体中部的面骨密度 ( L- MBMD)年变化率 6.19( 2 .45 ,11.88) %· a-1和侧位腰椎椎体中部的体积骨密度 ( L- MVBMD)年变化率 6.81( 2 .45 ,11.5 9) %· a-1。 ( 2 )各项骨密度值年变化率作相关分析 ,得到正位与侧位各项指标中度相关 ( r>0 .463,P<0 .0 0 1) ;侧位各项指标之间相关较好 ( r>0 .62 ,P<0 .0 0 1) ;侧位扫描面骨密度与其相应部位体积骨密度高度相关 ( r>0 .88,P<0 .0 0 1)。 ( 3)腰椎椎体中部松质骨的 L-MBMD和 L- MVBMD随访年变化率最高。 ( 4)腰椎质控模型的质控数据显示正侧位扫描模型 CV值分别为
OBJECTIVE: This study compares the measurement of bone mineral density at various parts of the lumbar spine to obtain a sensitive index to follow the annual rate of change of lumbar bone mineral density in patients with osteoporosis. Methods 54 female osteoporosis clinic patients were followed up with HOLOGIC QDR-2000 DXA bone densitometer. Age 5 8.4 ± 8.2 years old.
