目的:探讨超声弹性成像(ultrasonic elastography,UE)评价5分法对乳腺良恶性疾病的诊断价值。方法:对2008年6月-2009年2月330例(356个病灶)乳腺实性肿瘤患者进行超声检查,同时行UE。以手术病理为诊断金标准,评价UE评分标准的意义。结果:UE评分为1、2和3分者诊断乳腺良性病变的准确度分别为97.0、97.5和79.3;UE评分为4和5分诊断为恶性病变的准确度分别为77.8和100.0。采用UE5分标准诊断乳腺恶性肿瘤的敏感度和特异度分别为82.8和90.8。采用UE5分标准诊断直径≤1cm乳腺病变的敏感度和特异度分别为88.0和100.0,诊断直径>1cm乳腺病变的敏感度和特异度分别为82.5和87.8。结论:UE5分法有助于鉴别乳腺良恶性病灶,今后有待进一步研究。
Objective: To investigate the value of 5-point ultrasonic elastography (UE) in the diagnosis of benign and malignant breast diseases. Methods: From June 2008 to February 2009, 330 patients (356 lesions) with solid tumors of the breast underwent ultrasonography and UE at the same time. To surgical pathology as the gold standard, evaluation of the significance of UE scoring criteria. Results: The accuracy of the diagnosis of benign breast lesions with the scores of 1, 2 and 3 were 97.0, 97.5 and 79.3, respectively. The accuracy of the diagnosis of malignant lesions with the score of 4 and 5 were 77.8 and 100.0 respectively. The sensitivity and specificity of using UE5 standard to diagnose breast cancer were 82.8 and 90.8 respectively. The sensitivities and specificities of using UE5 standard to diagnose breast lesions ≤1cm in diameter were 88.0 and 100.0, respectively. The sensitivity and specificity of diagnosing breast lesions> 1cm in diameter were 82.5 and 87.8, respectively. Conclusion: UE5 score helps identify benign and malignant breast lesions, pending further study.
