背景与目的:骨桥蛋白(osteopontin,OPN)是一种分泌性磷酸糖蛋白,可以同整合素和CD44v结合,在肿瘤发生、发展、侵袭、转移等病理过程中起作用。本研究探讨血浆中OPN检测,在预测肝细胞肝癌(hepatocellularcarcinoma,HCC)术后转移复发中的意义。方法:病例组选取手术切除的肝细胞癌患者56例,用ELISA法检测术前血浆OPN浓度,对照组取23例健康体检职工血浆标本。比较病例组与对照组血浆OPN水平;并随访病例组复发转移情况,分析血浆OPN水平与肿瘤转移复发的关系。结果:HCC组血浆OPN浓度为(864.4±391.9)!g/L,健康对照组血浆OPN浓度为(382.5±169.4)!g/L,两组相比较存在显著性差异(P<0.001)。随访期间,25例无复发,31例复发。肝细胞癌无复发组血浆OPN浓度为(744.6±323.9)!g/L,复发组血浆OPN浓度为(961.0±411.6)!g/L,复发组与无复发组血浆OPN浓度比较有显著性差异(P=0.039)。Cox模型多因素分析发现血浆OPN水平、肝硬化、肿瘤大小是预测肝癌转移复发的独立因素。结论:肝细胞癌患者血浆中OPN浓度可以作为预测肿瘤复发转移的潜在参考分子指标之一。
BACKGROUND & OBJECTIVE: Osteopontin (OPN) is a secreted phosphoglycoprotein that binds to integrin and CD44v and plays a role in pathological processes such as tumorigenesis, development, invasion and metastasis. This study was designed to investigate the significance of detecting OPN in plasma to predict the metastasis and recurrence of hepatocellular carcinoma (HCC). Methods: Fifty-six patients with hepatocellular carcinoma who underwent surgical resection were selected in the case group. Preoperative serum OPN concentration was measured by ELISA and 23 healthy volunteers plasma samples were taken from the control group. The level of plasma OPN was compared between cases and controls. The recurrence and metastasis of cases were followed up. The relationship between plasma OPN levels and tumor metastasis and recurrence was analyzed. Results: The plasma OPN concentration in HCC group was (864.4 ± 391.9) g / L, while that in healthy control group was (382.5 ± 169.4) g / L. There was significant difference between the two groups (P <0.001). During follow-up, 25 had no recurrence and 31 had recurrence. The level of OPN in the relapse-free group was (744.6 ± 323.9) g / L, and the OPN concentration in the relapsed group was (961.0 ± 411.6) g / L, there was a significant difference between the two groups (P = 0.039). Cox model multivariate analysis found that plasma OPN levels, cirrhosis, tumor size is an independent factor in predicting the recurrence and metastasis of liver cancer. CONCLUSIONS: The concentration of OPN in the plasma of patients with hepatocellular carcinoma can be used as one of the potential molecular markers for predicting tumor recurrence and metastasis.
