目的探讨老年2型糖尿病患者周围神经病变的影响因素。方法回顾分析1 069例≥60岁的2型糖尿病患者资料,根据肌电图分为糖尿病周围神经病变(DPN)组和非糖尿病周围神经病变(NDPN)组,对两组数据比较,探寻DPN的影响因素。结果①DPN组873例(81.7%),NDPN组196例(18.3%);②DPN与NDPN两组的年龄分别为(71.20±6.70)岁、(69.54±5.92)岁,糖尿病病程分别为(12.42±8.02)年、(9.89±7.67)年,糖化血红蛋白(HbA1c)分别为(9.04±2.52)%、(8.26±1.91)%,性别比(男/女)分别为450/423、83/113,空腹C肽分别为(471.25±320.31)pmol/L、(529.72±298.10)pmol/L,总胆固醇(TC)分别为(4.93±1.27)mmol/L、(4.62±1.09)mmol/L,低密度脂蛋白胆固醇(LDL)分别为(2.95±0.90)mmol/L、(2.77±0.81)mmol/L,以上各指标差异有统计学意义(P<0.05或P<0.01);③Logistic回归结果显示,DPN与年龄、糖尿病病程、HbA1c、TC有关(OR值分别是1.047、1.042、1.152、1.275)。结论 DPN与老年2型糖尿病患者年龄、糖尿病病程、HbA1c、TC相关,联合控制血糖和血脂对于老年人周围神经病变的防治具有重要意义。
Objective To investigate the influencing factors of peripheral neuropathy in elderly type 2 diabetic patients. Methods One hundred and sixty-nine patients with type 2 diabetes mellitus (≥60 years old) were retrospectively analyzed. According to EMG, they were divided into two groups: diabetic peripheral neuropathy (DPN) group and non-diabetic peripheral neuropathy group (NDPN) Influencing factors. Results ① There were 873 cases (81.7%) in DPN group and 196 cases (18.3%) in NDPN group. (2) The differences between DPN and NDPN group were (71.20 ± 6.70) years old and (69.54 ± 5.92) years old respectively, and the duration of diabetes were (12.42 ± 8.02) (9.89 ± 7.67) years, respectively. The HbA1c was (9.04 ± 2.52)% and (8.26 ± 1.91)% respectively, and the sex ratio (male / male) was 450/423 and 83/113 respectively. (471.25 ± 320.31) pmol / L and (529.72 ± 298.10) pmol / L respectively, and total cholesterol (TC) were 4.93 ± 1.27 mmol / L and 4.62 ± 1.09 mmol / L respectively. The levels of low density lipoprotein (2.95 ± 0.90) mmol / L and (2.77 ± 0.81) mmol / L, respectively (P <0.05 or P <0.01); ③Logistic regression analysis showed that the correlation between DPN and age , Duration of diabetes, HbA1c and TC (OR = 1.047, 1.042, 1.152 and 1.275, respectively). Conclusion DPN is associated with age, duration of diabetes mellitus, HbA1c, TC in elderly patients with type 2 diabetes mellitus, and combined control of blood glucose and blood lipids in the prevention and treatment of peripheral neuropathy in the elderly.
