目的 比较经皮双侧膈神经电刺激和颈部磁波刺激法评价膈肌功能的简便性及其诱发的电、磁刺激颤搐性跨膈压 [Pdi(t)e、Pdi(t)m]的差异。方法 对 13例正常人和 12例慢性阻塞性肺疾病 (COPD)患者观察两种刺激方法的定位时间及其诱发的Pdi(t)e和Pdi(t)m。结果 2 2例非机械通气者电刺激法的定位时间长于颈部磁波刺激法 (P <0 0 1) ;正常组的Pdi(t)m高于Pdi(t)e(P <0 0 1) ,但两者接近且明显相关 (r=0 92 ,P <0 0 1) ,COPD组的Pdi(t)m与Pdi(t)e无统计学差异 (P >0 0 5 ) ;两组Pdi(t)m的变异系数均低于Pdi(t)e(P <0 0 5 )。结论 颈部磁波刺激法测定膈肌功能优越于电刺激法。
OBJECTIVE: To compare the efficacy of percutaneous bilateral phrenic nerve stimulation with cervical magnetic stimulation in assessing the diaphragmatic function and the induced electrical and magnetic stimulation of twitch-cross diaphragmatic pressure [Pdi (t) e, Pdi (t) m] difference. Methods The localization time and induced Pdi (t) e and Pdi (t) m of the two stimulation methods were observed in 13 normal subjects and 12 patients with chronic obstructive pulmonary disease (COPD). Results Electroacupuncture of 22 patients with non-mechanical ventilation had longer positioning time than magnetic stimulation of the neck (P <0.01), Pdi (t) m of the normal group was higher than that of Pdi (t) e (P <0.01) (P <0.01). There was no significant difference between Pdi (t) m and Pdi (t) e in COPD group (P> 0.05) (t) m were lower than Pdi (t) e (P <0 05). Conclusion Magnetic stimulation of the neck is superior to electrical stimulation in the determination of diaphragmatic muscle function.
