目的 探讨 Konno手术在儿童主动脉瓣环狭窄和心瓣膜置换中的应用价值。 方法 应用 Konno法 ,人工血管补片扩大室间隔 ,主动脉瓣环植入 2 1#机械瓣 ,用同一人工血管补片扩大升主动脉 ,再另取一人工血管补片扩大右室流出道。 结果 2例患者均成活 ,无残余杂音 ;主动脉瓣口跨瓣压差分别从术前的 12 7mm Hg(1k Pa=7.5mm Hg)降至术后的 2 3mm Hg,75 mm Hg降至 15 mm Hg。 1例并发伤口感染 ,另 1例并发右心功能不全 ,均经治疗痊愈。结论 Konno手术是治疗儿童主动脉瓣环狭窄较理想的方法 ,在儿童主动脉瓣置换中亦能应用 ,掌握其技术要点可取得较好的效果。
Objective To investigate the value of Konno’s surgery in the treatment of children’s aortic annulus stenosis and heart valve replacement. Methods The Konno method was used to expand the interventricular septum with artificial blood vessel patch. The aortic annulus was implanted with 21 # mechanical valve, the same artificial blood vessel patch was used to expand the ascending aorta, and another artificial blood vessel patch was used to expand the right ventricular outflow tract. Results All the 2 patients survived with no residual noise. The aortic valve transvalvular pressure drop decreased from 12 7mm Hg (1k Pa = 7.5mm Hg) to 23mm Hg postoperatively and 75 mm Hg to 15 mm Hg. One case complicated with wound infection, the other one complicated by right ventricular dysfunction, were cured after treatment. Conclusion Konno operation is an ideal method for the treatment of pediatric aortic annulus stenosis. It can also be used in pediatric aortic valve replacement. To master the technical points of Konno operation can achieve better results.
