目的评价18F脱氧葡萄糖(FDG)PET心肌葡萄糖代谢与99mTc甲氧基异丁基异腈(MIBI)心肌灌注显像相结合对存活心肌的诊断价值。方法90例临床确诊为陈旧性心肌梗塞(OMI)患者,男80例,女10例,平均年龄558±97岁。所有患者行心肌灌注显像和心肌代谢显像,均做超声心动图心功能测定,其中76例做冠状动脉造影。血运重建术后,36例复查超声心动图,测定心功能(A组);24例复查心肌灌注显像及超声心动图(B组)。心肌代谢显像采用国产PETB03系统。结果90例患者心肌灌注显像有397个灌注减低节段,结合代谢显像将其分为:灌注代谢不匹配(MM)节段202个,占509%,提示心肌存活(A2和B2组);心肌灌注代谢匹配(M)139个节段,占35%,提示心肌梗塞(A1和B1组);灌注代谢部分不匹配(PM)56个节段,占141%,提示心肌部分存活。18FFDGPET心肌代谢显像与灌注显像相结合对室壁运动和心肌灌注改善的阳性预测值分别为875%(42/48)和838%(57/68),阴性预测值分别为714%(25/35)和846%(22/26),预测准确率分别为800%和841%。以?
Objective To evaluate the diagnostic value of myocardial glucose metabolism in myocardial infarction with 18Fdeoxyglucose (FDG) PET myocardial perfusion imaging and 99mTcmethoxyisobutylisonitrile (MIBI) myocardial perfusion imaging. Methods Ninety clinically diagnosed patients with obstructive myocardial infarction (OMI), 80 were male and 10 were female, with an average age of 558 ± 97 years.
